  Observation. One child's file did not contain the name and telephone number of a medical practitioner.

  Observation. Two staff files did not have a record of immunization.

  Observation. One staff certificate was not validated through the ECE Registry.

  A licensee must keep, for each child, a record showing the following information: (d) name and telephone number of a parent, medical practitioner and emergency contact.

  Observation. A review of 6 children's records indicated that two were missing emergency contact information. One was missing Doctors Name or Clinic and phone number.

  Corrective action. Ensure emergency contact information and Doctor or Clinic information and phone numbers are available for each child in care.
