A licensee must keep, for each child, a record showing the following information: (a) name, sex, date of birth, medical insurance plan number and immunization status.

Observation. Regarding the children's files for the GCC (30 months to school age) program: - Health number not available on file for one child - Immunization status record missing for two children

Corrective action. Ensure children's files are complete prior to starting care at the facility.

Observation. In the GCC (under 36 months) program: - One child did not have the doctor's phone number on file In the GCC (30 months to school age) program: - One child did not have the doctor or clinic's name and phone number available on file - No emergency contact available for one child

Corrective action. Ensure children's files are complete prior to starting care at the facility.
