    Observation. Licensing observed the following: a. Change mat in the bathroom is ripped b. 3 shelves located in the main play room are taller than 3 ft. and are not secured.

    Corrective action. Supervisor stated the following: a. Supervisor will replace the change mat b. Supervisor will secure shelves

    A licensee must ensure that the entire outdoor play area is (b) constructed in a manner, and using materials, that are suitable for the age and development of the children intended to use it.

    Observation. Licensing reviewed 9 children's files and the following was noted: a. 2 files were missing enrolement date b. 2 files were missing an emergency contact c. 1 file was missing the name and phone number of a doctor d. 1 file was missing an MSP number

    Corrective action. Supervisor stated she will review the files and connect with the families that have missing information and ask them to update the file so that it is complete and up to date.

    A licensee must ensure that children do not have access to any object or substance that may be hazardous to the health or safety of a child. Observation (CORRECTED DURING INSPECTION): Upon inspection, Licensing found diaper creams with din #'s accessible to children in the bathroom.

    Corrective action. The Manager moved these during the inspection.

    Observation. Upon inspection, Licensing observed an individual watering the yard and a criminal record check was not obtained.

    Corrective action. The Manager explained that this individual lives at this location and will follow up with the owner.
