A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.
Observation. Licensing observed a pool of standing water approximately 2 inches in depth. Manager said that the pool of water was left from water day. The pool was fenced of and the children could not have access to it.
Corrective action. Manager said that they will remove the pool and water.

Observation. A heavy chalk board was observed leaning on the fence ins the outside play area
Corrective action. Manager moved the chalk board out of the outdoor play area till the permanent solution to use it is in place

Observation. Licensing reviewed 5 children's records and noted 2 did not contain the date of enrolment at the facility.
Corrective action. Please ensure children's records contain all required information, including the date of enrolment.

A licensee must keep, for each child, a record showing the following information: (e) any illness, allergy or medical disability disclosed to the licensee by the child or his or her parent or medical practitioner.
Observation. Licensing observed that care plan has not been developed and included in child's file.
Corrective action. Ensure that a care plan is developed in discussion with the parent for any illness or allergies.

A licensee must keep, for each child, a record showing the following information: (d) name and telephone number of a parent, medical practitioner and emergency contact.
Observation. One of the children's files had a medical practitioner's phone number and the emergency contact information missing.
Corrective action. Ensure children's registration information is complete with all required documentation including emergency contacts and doctor's phone number.

Observation. Tow of the three files reviewed did not include the copy of the staff resume.
Corrective action. Ensure that the copy of the resume is obtained and included in the staff files.

Observation. One of the three staff files reviewed showed that the immunization status was not obtatined.
Corrective action. Ensure staff screening and required documentation is complete priro to the employment and is inclded in the file.

Corrective action. Ensure that the certificate is validated and the copy of the validation is included in the staff file.

Observation. One of the three staff files reviewed showed that the certificate was not obtained and included in the file.
Corrective action. Ensure that the copies of certificate is obtained and included in the file prior to the employment.
