A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.

Observation. Upon inspecting the outside area licensing noted that the fence had 4 inches gaps (slots) - DLSOP Safe Play Space – Spaces (i.e. holes or openings) are smaller than 9 cm (3.5 inches) or larger than 23 cm (9 inches). This poses a risk for potential entrapment.

Corrective action. Licensee will look in adding privacy slots and covering the gap in the fence slots.

A licensee must keep current records of each of the following: (d) written policies and procedures that are intended to guide employees in the care and supervision of children.

Observation. DLSOP Active Play – The Licensee has not developed and/or implemented a Screen Use Policy and Active Play Policy.

Corrective action. Licensee said that they will create a Screen Time Policy and an Active Play Policy.

If a person prepays part of the cost of services provided by a class of community care facilities designated by the Lieutenant Governor in Council, the licensee must, at the time of the prepayment, deliver to the person a written statement setting out the terms and conditions on which a refund of all or any of the prepayment will be made.

Observation. The Licensee did not provide a written statement outlining the terms and conditions of refund to a person at the time of prepayment.

Corrective action. Licensee said that they will create a Repayment Policy.

A licensee must keep current records of each of the following: (e) written policies and procedures respecting food and drink to be given to children.

Observation. Food and Nutrition policy was not available.

Corrective action. Licensee said that they will create a Nutrition Policy.

A licensee must keep, for each child, a record showing the following information: (b) date of enrolment in the community care facility. Observation (CORRECTED DURING INSPECTION): Licensing reviewed 13 children's files and 6 were missing a date of enrollment. This is a repeat contravention from 2023 and 2025.

Corrective action. Licensee corrected the viloation while licensing was on site.

A licensee must not employ a person in a community care facility as (a) an educator or an assistant unless the person holds a certificate issued under Division 2 [Employee Qualifications], which must be verified by the system maintained by the early childhood educator registry. (Educator or assistant certificate validation) Observation (CORRECTED DURING INSPECTION): Licensing reviewed 5 staff files and one staff file was missing validation of ECE certificate.

Corrective action. Staff corrected the violation while licensing was on site.

A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (a) a criminal record check for the person.

Observation. Licensing reviewed 5 staff file and observed that a Criminal Record Check was not obtained for one staff. This is a repeat contravention from 2025.

Corrective action. Ensure all files are complete.

A licensee must: (b) have an emergency plan that sets out procedures to prepare for, mitigate, respond to and recover from any emergency.

Observation. Emergency supplies kit had musty odour to it and was not up to date as it had some expired food and not suffecient supplies to mitigate and recover from any emergency

Corrective action. Licensee will update the supplies kit and will store it near an exit

A licensee must keep, for each child, a record showing the following information: (b) date of enrolment in the community care facility.

Observation. Start date was missing in children's files.

Corrective action. Discussion around registration date and start was had with the licensee. Licensee will complete the registration forms to reflect the start date.

A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (a) a criminal record check for the person.

Observation. Criminal Record Check was not obtained for one staff prior to her starting to work.

Corrective action. Licensee has applied for CRC and is in the process to obtain it. Licensee will try to obtain a copy of the CRC from school in the meantime

A licensee must ensure that children have at all times immediate access to an employee who (a) holds a valid first aid and CPR certificate, provided on completion of a course that meets the requirements of Schedule C.

Observation. First Aid certificate for one of the staff was completed online

Corrective action. Licensee will check to see that staff is not working with the children alone or renew the first aid course that meets the requirements of Schedule C

A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (b) character references in respect of the person.

Observation. A minimum of two character references were not obtained for one or more employees.

Corrective action. Licensee will obtain the character references and include in the file for the staff.

A licensee must ensure that each employee (b) practices implementing the fire drill system at least once each month.

Observation. The record of practicing fire drills for School Age program is not being maintained.

Corrective action. Ensure that the fire drills are practiced monthly and the record is maintained

A licensee must keep, for each child, a record showing the following information: (a) name, sex, date of birth, medical insurance plan number and immunization status.

Observation. Upon review of the children's files licensing observed the record of medical insurance number and immunization status was missing.

Corrective action. Ensure that for each child a record showing medical insurance number and immunization status is maintained in each of child's files.

A licensee must keep, for each child, a record showing the following information: (b) date of enrolment in the community care facility.

Observation. Children's records did not have date of enrollment recorded.

Corrective action. Ensure that for each child a record showing the date of enrollment is included in the file.

A licensee must keep, for each child, a record showing the following information: (c) daily attendance record, indicating for each day whether the child is absent or, if the child is present, the time of arrival and departure.

Observation. The time of departure was not documented on many days throughout the month of review on the Preschool attendance records.

Corrective action. Ensure that the system is created to accurately reflect children in attendance to include both arrival and departure.

A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (b) character references in respect of the person.

Observation. Upon review of staff files licensing observed that the character references were not obtained or the record included in the staff file.

Corrective action. Ensure that two references are obtained and record is included in the staff files.

A licensee must ensure that each employee (b) practices implementing the fire drill system at least once each month.

Observation. The record of practicing fire drills for past two months was missing and staff was aware of it.

Corrective action. Ensure that the fire drills are practiced monthly and the record is maintained.

To qualify for employment in a community care facility as a responsible adult, a person must (c) have completed a course, or a combination of courses, of at least 20 hours duration in child development, guidance, health and safety, or nutrition.

Observation. Upon review of staff files licensing observed that two staff files did not have evidence of 20 hours of relevant courses.

Corrective action. Ensure that the staff has completed at least 20 hours of relevant course/courses prior to the employment.

A licensee must keep, for each child, a record showing the following information: (b) date of enrolment in the community care facility.

Observation. Children's records did not have date of enrollment recorded.

Corrective action. Ensure the children's records show the date of enrollment on registration forms.

A licensee must keep, for each child, a record showing the following information: (d) name and telephone number of a parent, medical practitioner and emergency contact.

Observation. Children's records did not have telephone number of the medical practitioner.

Corrective action. Ensure each child has medical practitioner information in their file

A licensee must keep, for each child requiring extra support, a current care plan showing the following information: (a) the diagnoses relevant to the child's requirement for extra support, as made by health care professionals.

Observation. No care plan available for a child requiring extra support.

Corrective action. Ensure a current care plan is available for the child requiring extra support

A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (b) character references in respect of the person.

Corrective action. Ensure to obtain character references for each employee and the record is maintained in the file.

Corrective action. Ensure that prior to employment each employee has a criminal record check obtained and the copy is available in the staff file.

A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (f) evidence that the person has complied with the Province's immunization and tuberculosis control programs.

Observation. Immunization status was not obtained for one or more employees.

Corrective action. Ensure each employee has an immunization status and the copy is available in the staff files

A licensee must keep, for each child, a record showing the following information: (c) daily attendance record, indicating for each day whether the child is absent or, if the child is present, the time of arrival and departure.

Observation. At review of attendance system it was not captured if a child was away / not present.

Corrective action. Please include whether a child is present or absent within your attendance record.

A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (f) evidence that the person has complied with the Province's immunization and tuberculosis control programs.

Observation. One of three staff files reviewed missing immunization status.

Corrective action. please ensure immunization status is available for review
