  Observation. A sample of children records were reviewed. Several children did not had a doctor contact and/or the medical insurance plan number on file Children records were stored electronically and in a paper folders.

  Observation. Licensing officer was unable to observe CRC,work history ,references and verification of the certificates. The manager stated that the HR is responsible for those records.
