Observation. Licensing observed the following: 1) The door to the laundry room / closet was unsecured. The means of securing the door and preventing child access was flimsy. 2) The door to the master bedroom was unsecured. The master bedroom was cluttered and unsafe.

Observation. Licensing observed the following: 1) Tools such as hammers, screwdrivers, etc. in an unlocked kitchen drawer. 2) Uncovered electrical outlets in the hallway across the bathroom and in the main activity space beside the kitchen counter.

Observation. The gates on the side of the house and stairs to the basement were unlocked. The gate on the side of the house was left wide open. The Licensee stated that the basement tenant leaves the gates unsecured. The Licensee stated they check the gates when they go outside to play. Licensing observed a table against the fence located beside steps leading to the garage. A child could easily climb from the steps on top of the table and over the fence.

Maintenance of Equipment: Thorough inspections of the outdoor play space are conducted regularly. [Director of Licensing Standard of Practice - Safe Play Space] (13)
Observation. The Licensee does not conduct and record outdoor play space inspections. The Director of Licensing Standards of Practice requires the following: Thorough daily, monthly and annual inspection programs must be performed regularly. Checklists will assist in reminding staff of the types of checks to be performed and in ensuring that proper maintenance and repair are completed. Daily: check the play space for garbage, broken glass, animal feces and other hazardous items. Monthly: check the play space in greater detail, including a check of the landscaping and fencing and overall condition of equipment to ensure that they do not pose any hazards to children. Annually: conduct a detailed audit of the play space, landscaping, and all equipment.

Observation. The uneven concrete in the backyard has been addressed. Licensing walked around the side of the home into the backyard to see if the children were outside. Licensing observed that the gate latch was falling off as the screws were loose into broken wood.

Observation. Licensing observed the following: * the kitchen counters appeared to be cluttered with a variety of items such as dishes, food and papers. The Licensee was preparing food on the counters. * a platter on the counter that contained fruit, also had other items such as electrical plugs, cords, receipts, pieces of paper, pens, markers, and paperclips. * the potty the children are using was not cleaned and sanitized.

Licensing recommends power washing the toys and equipment. * the strollers were rusty, dirty and one had a wheel that was broken. These were accessible to the children. * the rubber mats in the outdoor play area were uneven and had pooling water.

Licensing observed that the gate on the deck was in bad repair, it no longer closes and latches, the wood is starting to splinter in some areas and there are nails that are accessible to the children. Licensing also observed that the stairs and railings are also in bad repair. Some areas the wood is rotting and the splintering, the railings are loose and the rubber on the stairs is pealing off in numerous places.
