    Observation. The lock/latch on the door in between the main activity space and patio is jammed, and does not close/open properly. A work order has been submitted.

    Observation. A large hole has appeared on the side of the sandbox area. Manager informs that a work order has been sent in already. Continue monitoring other areas of rotting wood stumps, and plan for replacement. Facility has placed rubber tires across several rotting wood stumps, and plan to grow plants/vegetables inside so children do not run on the area or climb there.

    Observation. Licensing observed rotting wood planks and wooden stumps in several areas of the playground. Manager informs that a work order has been submitted for repairs. In the meantime, a supervision plan is put in place for areas such as the sandbox, and temporary reinforcements for rotting wood stumps with rubber tires as gardening beds. Conversation surrounding the cement base of the post on the hill being covered or padded in case of fall accidents from the slope. roots protruding from the dirt by surrounding vegetation may cause tripping hazards. Manager informs these can be cut.

    Observation. Licensing observed a paper towel dispenser next to the cubby area, to be held together by tape. This item is no longer in good repair. Children are currently using a different holder for paper towel after washing hands. Manager informs that this dispenser is not being used, and can be removed.

    Observation. *During today's visit a child was observed to be eating snack; while there was an educator present, they were observed to be busy at times with other tasks such as greeting children and writing notes in the communication book with their back turned.

    Observation. *Licensing observed unsecured and accessible blind cords throughout the facility (main activity areas and nap room). These pose a possible entanglement hazard. *A string of lights in the nap room was hanging down, loose and accessible. This poses the possibility of entanglement.

This is a slip hazard. *The climbing wall on the outdoor climber is rotting all along the bottom. This item is no longer in good repair. *Licensing observed several stumps and many of the wooden features in the outdoor play yard are rotting and have become deteriorated.
