Observation. Immunization status was not on file for the Licensee.
Corrective action. Licensee will fill out immunization declaration and send it to Licenseing.

A licensee must: (b) have an emergency plan that sets out procedures to prepare for, mitigate, respond to and recover from any emergency.
Observation. Licensing observed that the emergency supplies had no food and a few water bottles in the kit. The licensee will purchase more supplies and ensure there is sufficient amount for herself and each child for 72 hours.
Corrective action. Please ensure that the emergency supplies kit is updated and checked frequently.
13020 - CCLR s.22(1)(b) - POLICIES AND PROCEDURES
A licensee must keep current records of each of the following: (c) a record respecting compliance with section 22 (2) (b) and (c) [emergency training and equipment].
Observation. Licensing observed that the emergency drill was not recorded for 2023/2024. The licensee said she will practice the drill before the end of this year.
Corrective action. Please ensure the evacuation emergency drill is practiced and recorded annually.

A licensee must keep, for each child, a record showing the following information: (d) name and telephone number of a parent, medical practitioner and emergency contact.
Observation. Licensing reviewed 4 children's files; 3 children did not have medical practitioner's name and telephone number.
Corrective action. Please ensure children's files are up to date and there is a process to check them.

A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.
Observation. The following was observed in the outdoor play space used by children: -tall bucket full of water -access to stairs -vehicle tires leaning against the shed The following was observed in the kitchen: -small knife in the drying rack - removed during the inspection -9 volt battery that appeared corroded on the counter - removed during the inspection
Corrective action. Licensee stated the outdoor play area will be addressed to meet compliance.

A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.
Observation. Licensing observed a high chair, that a child uses, that is dirty with brown stains on it and several crumbs on it. Licensing provided education on cleaning and sanitizing all equipment after a child as used it.
Corrective action. Ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair.
11020 - CCLR s.13(2) - EQUIPMENT AND FURNISHINGS
A licensee must ensure that children do not have access to any object or substance that may be hazardous to the health or safety of a child.
Observation. Licensing observed the following that are accessible to children in care; - A tall heating lamp in the cement outdoor play area. Potential for a child to pull or bump into it and fall on them and injure them. - A diaper rash ointment in one of the buckets on the diaper change table.
Corrective action. Ensure that children do not have access to any object or substance that may be hazardous to the health or safety of a child.
11200 - CCLR s.17 - POLICIES AND PROCEDURES
A licensee must keep current records of each of the following: (c) a record respecting compliance with section 22 (2) (b) and (c) [emergency training and equipment].
Observation. Licensing observed that there was no record of a fire drill implemented for the month of October 2022. The Licensee informed Licensing that a fire drill was practiced but not recorded.
Corrective action. Ensure that current records are kept for fire and emergency drills.

Observation. Licensing observed that only one of the two sleeping mats that are ripped has been addressed by placing a plastic cover over the mat. The Licensee informed Licensing that a cover for the second mat is in the storage area and will be placed on the mat today, Jan.10/22.
Corrective action. Please ensure that all the furniture, equipment and fixtures within the community care facility are clean and in good repair.

- EQUIPMENT AND FURNISHINGS
A licensee providing a care program to any child younger than 36 months of age must provide a separate sleeping area located away from any activity area.
Observation. Licensing observed that the nap room/area has been turned into the Licensee's personal gym and has several gym equipment in it. The Licensee informed Licensing that the children sleep in the main play area. Licensing provided education on the intent of the regulation.
Corrective action. Please ensure that a care program that provides care to any child younger than 36 months of age must provide a separate sleeping area located away from any activity area.
11130 - CCLR s.15(3) - EQUIPMENT AND FURNISHINGS
A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.
Observation. Licensing observed the following that are accessible to children in care; - A metal ladder in the outdoor grass play area. - A long wood piece, on the outdoor grass area, resting against the wooden fence. - Several long sharp branches coming over the fence into the outdoor play area. - An industrial paper cutter on the table in one activity play room. - One tall shelf in one of the activity play area and one tall cupboard in the kitchen that are not secured to the wall. - An unlocked cupboard in the kitchen with several Lysol wipes containers.
Corrective action. Please ensure that a healthy and safe environment is provided at all times while children are in care.
11010 - CCLR s.13(1) - EQUIPMENT AND FURNISHINGS
A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.
Observation. Licensing observed two sleeping mats ripped in several areas exposing the foam underlay. Licensing provided education that if the mats are ripped then effective sanitization is not accomplished.
Corrective action. Please ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in care.
11020 - CCLR s.13(2) - EQUIPMENT AND FURNISHINGS
A licensee must supply equipment, furniture and supplies that are (b) located so as not to block or hamper an exit in the case of fire or other emergency.
Observation. Licensing observed a large card box located in front of the hallway. Where the box is situated causes for a very tiny space to get through and hampers the hallway leading to the emergency exit. The Licensee informed Licensing that the box is there because the children were painting it. Licensing provided education on the importance of not placing any furniture or equipment anywhere where it would block or hamper an exit in the case of any emergency.
Corrective action. Please ensure that all equipment, furniture and supplies do not block or hamper an exit in the case of fire or other emergency.
11090 - CCLR s.15(1)(b) - POLICIES AND PROCEDURES
A licensee must keep current records of each of the following: (c) a record respecting compliance with section 22 (2) (b) and (c) [emergency training and equipment].
Observation. Licensing observed that there was no evidence of an emergency drill being implemented in the past year. The Licensee informed Licensing that an emergency drill was practiced but not recorded. Licensing provided education on the intent of the regulation and that all fire and emergency drills are required to be documented.
Corrective action. Please ensure that current records are kept in respect to section 22(2)(b) and (c).

A licensee must keep, for each child, a record showing the following information: (d) name and telephone number of a parent, medical practitioner and emergency contact.
Observation. Upon a random review of four children's records, Licensing observed one child's record missing the name and telephone number of a medical practitioner.
Corrective action. Please ensure that children's records are complete prior to the child attending the community care facility.

A licensee must keep, for each child, a record showing the following information: (i) a photograph or digital image of the child, and other information that can be used to readily identify the child in an emergency.
Observation. Upon a random review of four children's records, Licensing observed one child's record missing a photograph or digital image of the child.
Corrective action. Please ensure that children's records are complete prior to the child attending the community care facility.
19160 - CCLR s.57(2)(i) - RECORDS AND REPORTING
A licensee must have in writing from a parent, and maintain at the community care facility, consent (a) to call a medical practitioner or ambulance in case of accident or illness if the parent cannot immediately be reached.
Observation. Upon a random review of four children's records, Licensing observed one child's record missing a parent's consent to call a medical practitioner or ambulance in case of an accident or illness if the parent cannot immediately be reached.
Corrective action. Please ensure that children's records are complete prior to the child attending the community care facility.
19190 - CCLR s.57(3)(a) - RECORDS AND REPORTING
A licensee must keep current records of each of the following: (f) a log of minor accidents, illnesses and unexpected events involving children, that did not require medical attention and were not reportable incidents described in Schedule H.
Observation. The Licensee informed Licensing that they have misplaced the minor incident log book and a new one has not been purchased to replace it. Therefore, there is no minor incident log book available, new or old, for Licensing to review. Licensing provided education on the importance of having a minor incident log book available and maintaining it.
Corrective action. Please ensure that a minor incident log book is kept and maintained.
19050 - CCLR s.56(1)(f) - STAFFING
A licensee must ensure that each employee (b) practices implementing the fire drill system at least once each month.
Observation. Licensing observed that fire drills were not implemented for the months of July, August and November 2021.
Corrective action. Please ensure that fire drills are implemented at least once each month.
12190 - CCLR s.22(2)(b) - STAFFING
A licensee must keep current records of each of the following: (b) for each employee, the records required under section 19 (1) [character and skill requirements].
Observation. Licensing observed that the Licensee's criminal record check has expired. The Licensee has applied for a new criminal record check a week ago. Licensing suggested having a system in place to ensure that criminal record checks are applied for early so that they arrive prior to the expiry date.
Corrective action. Please ensure that staff records are current.

Observation. The backyard was inspected and the following was observed: - a small section of the fence is open and there is a gate that is leaning up against it but does get knocked over by the wind -licensee stated that a more permanent gate will be installed so children do not have access to the side of the house -there are large pieces of plywood leaning up against the shed that were left over when the new fence was put in -licensee stated that those pieces of wood will be moved so children do not have access
Corrective action. Please ensure the backyard is free from hazards and in good repair
