  Observation. Through the incident report submitted to Licensing on October 11, 2024 and through discussion with the facility manager, it is apparent that a child was left outside alone for a period of time. Submit to Licensing a policy that will ensure staff are accounting for all children in the program during transitions to and from the outside play area.

  Observation. The incident reported to Licensing on October 11, 2024 involving a child was not reported immediately to the parent of that child.

  Observation. Observed the sleep mats are stored touching each other, and are not being sanitized daily.
