    Observation. It was discovered in relation to the complaint that an incident that involved an employee using restraint as a form of behaviour guidance for a child. The restraint was used to mange the child's behaviour and does not qualify as an emergency restraint as the child was not in imminent serious physical harm. The child does not have a care plan in place. The use of restraint is only authorized in the event a child is at risk of imminent serious physical harm or is outlined in a child's care plan for the health and safety of the child while in care. A response is required by March 11, 2026 that outlines how the risk has been mitigated.

  • C4.1 - Are facility records current and complete?Non-critical

    Observation. It was discovered in relation to the complaint that an incident occurred at the facility involving unusual or aggressive behaviour by a child that resulted in the use of an emergency restraint and was not reported to Licensing as required. It is the responsibility of the Licensee to ensure Licensing is notified within 24 hours of a reportable incident. Submit the reportable incident as required by March 11, 2026. It was alleged that an inappropriate plan for a child was developed by the facility as a result of a reportable incident to mange the child's behaviour. The plan was reviewed and does not contain the relevant information as required in a care plan for a child who requires extra support. It is the responsibility of the Licensee to ensure a care plan is developed for any child who requires extra support for the health and safety of the child while in care. Substantiated. Submit a response by March 11, 2026 that outlines how care plans will be developed as required.

    Observation. A group of children were playing in a corner of the outdoor play space. The two employees were observed sitting in lawn chairs beside each other looking at their cell phones and towards the street away from the direction the children were playing. Children need to be actively supervised at all times. Outline the system developed to ensure active supervision is occurring during outdoor play.
