  • C10.2 - Are children supervised, and are employee to children ratios maintained?Critical

    Observation. Both incidents appear to involve a lack of supervision by employees. The Manager confirmed via correspondence that in both incidents inappropriate supervision may have contributed to the incidents occurring. One child was not being closely supervised while using offsite exercise equipment, and another child was left alone and unsupervised in the outdoor play space. Submit by June 13, 2025, confirmation that employees are providing children with continuous supervision.

  • C2.1 - Are written policies and procedures in place to guide employees in all matters regarding the care and supervision of children?Non-critical

    Observation. A reportable incident was received regarding a child falling off a piece of exercise equipment while at an offsite location, causing the child to sustain a broken arm. The Manager confirmed via correspondence that employees were not following the supervision policy by closely supervising the child at the time the incident occurred. If employees were implementing the supervision policy, they may have been able to stop the children from using the offsite play equipment in an unsafe manner. Submit by June 13, 2025, confirmation and evidence that employees are implementing the supervision policy. A reportable incident was received regarding a child being left in the facility’s outdoor play space alone without supervision. The Manager confirmed via correspondence that a parent found the child alone in the playground and brough the child to staff. If employees were implementing the supervision policy, they may have noticed the child hiding in the tunnel and been able to ensure the child was not left unsupervised. Submit by June 13, 2025, confirmation and evidence that employees are implementing the supervision policy.

  • C4.1 - Are facility records current and complete?Non-critical

    Observation. The Manager advised via correspondence that a child had not been signed in, therefore the number of children in care was incorrect. Due to this oversight, a child was left outside alone without supervision. The group transitioned inside without realizing a child had been left outside in a play tunnel. Not having accurate attendance records caused a child to be missed during their headcount. Submit by June 13, 2025, confirmation and evidence that child attendance records are accurate, and employees know the number of children they have in care.

  • C8.1 - Are children offered a variety of activities and instructed on the safe use of the play space?Non-critical

    Observation. Further to the incident of the child sustaining a broken arm; the Manager confirmed via correspondence that the child was not provided with suitable guidance on the use of the offsite equipment. The Licensing Officer observed the offsite play space and determined the community exercise equipment should be used in a specific manner to be safe. A child being allowed to spin on a chair designed for twisting would not be considered correct use of the equipment. Children require guidance and instruction on the safe use of equipment. Submit by June 13, 2025, confirmation on how children will be kept safe while using equipment while off-site.
