A record for each child containing the date of enrolment in the facility is not kept. [CCLR Sec. 57(2)(b)]; A record for each child containing the name and telephone number of a parent, medical practitioner and emergency contact is not kept. [CCLR Sec. 57(2)(d)]; A record for each child containing the name, sex, date of birth, medical insurance plan number and immunization status is not kept. [CCLR Sec. 57(2)(a)]

Observation. LO reviewed a sample of 8 children's files and observed the following: • 3 children did not have PHN/Care Card information on file • 1 child did not have a date of enrolment on file • 2 children did not have an emergency contact on file • 2 children did not have immunization status on file • 1 child did not have emergency consent on file, and 1 child had an unsigned emergency consent card on file
