    A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.

    Observation. Multiple baskets that are used to store children's toys on the shelves are torn.

    Corrective action. The Licensee will replace the baskets and will conduct regular checks of equipment to ensure they are in good condition.

    A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (b) character references in respect of the person.

    Observation. One staff is employed and the staff member did not have record of character references.

  • EQUIPMENT AND FURNISHINGS

    A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.

    Observation. During inspection licensing observed the following in the indoor play area: -An open garbage container with kitchen waste was within children's reach. -The Manager's personal purse was on the playroom floor. -Boxes were stored on top of a shelf, posing a risk of falling during an earthquake. The Manager removed the garbage container and purse during the inpsection, placing them out of children's reach.

    Corrective action. The Manager said the kitchen garbage container will be stored inside the kitchen cupboard, the purse will be kept out of children's reach, and the items on top of the shelf will be relocated to the garage.

  • EQUIPMENT AND FURNISHINGS

    A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.

    Observation. Licensing observed the following indoors: -Play floor mat has sections with ripped edges, showing signs of disrepair and exposing foam underneath, which makes it difficult to sanitize. -One edge of a wooden shelf within children's reach is broken, with peeling tape exposing the rough area underneath. -All cardboard building blocks have worn surfaces with pieces missing at the edges.

    Corrective action. Manager said these items will be either repaired or replaced.

  • RECORDS AND REPORTING

    A licensee must keep, for each child, a record showing the following information: (d) name and telephone number of a parent, medical practitioner and emergency contact.

    Observation. Four children's files were reviewed; one file included the name of the doctor and emergency contacts but was missing their phone numbers.

    Corrective action. The Manager said parents will be contacted to obtain the missing phone numbers and child's file will be updated.

  • EQUIPMENT AND FURNISHINGS

    A licensee must ensure that children do not have access to any object or substance that may be hazardous to the health or safety of a child. Observation (CORRECTED DURING INSPECTION): During inspection Licensing observed the following that were accessible to children: ·In the bathroom one can of Febreeze was stored on top of the toilet's water tank. Licensee removed this during inspection. ·A glass with tea was placed on the table where a child was eating snack. This was within child’s reach. Licensee from the other facility stated that they had placed it there when they heard Licensing speaking to the Licensee of this facility. Licensee removed this during inspection. ·A staff purse was placed on the table right beside a child who was eating snack at this table. The contents of this purse were visible and accessible to this child. Licensee removed this during inspection.

    Corrective action. Ensure that children do not have access to any object or substance that may be hazardous to the health or safety of a child.

  • EQUIPMENT AND FURNISHINGS

    A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.

    Observation. Licensing observed the following were acccesible to children in the main play area: -One end of the string attached to a long garland of balloons pinned to the wall with a thumb tack and the other end of this garland along with its attached string laying on the floor where children were playing. Licensing was concerned that the loose string can pose a strangulation hazard and balloons can pose a suffocation hazard. Licensee did not notice this till Licensing brought it to their attention. While Licensee was in the process of fixing the loose end onto the wall, the thumb tack previously attached to the wall came loose and the garland of balloons along with this thumb tack fell to the floor. This was removed during inspection. Balloons posing as a suffocation hazard for young children was discussed with the Licensee during the follow-up inspection # SMIA-C6QPVQ dated September 10, 2021. -Electric wire used for charging the cell phone was accessible to children. Licensee of the other facility removed it during inpection. - Tall play kitchen was not secured to the wall and was tippable on Licensing's touch. Licensing expressed concerns of the potential for this furniture to fall on a child. Licensee stated that a family member will secure this by today Janaury 17,2022.

    Corrective action. Please ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.

  • EQUIPMENT AND FURNISHINGS

    A licensee must ensure that each piece of furniture or equipment for sleeping (c) is appropriate for the size, age and development of the children intended to use it.

    Observation. During inspection Licensing noticed that there was an infant sleeping on a folded blanket which was placed on the carpet in the main play area. Licensing was not aware that an infant was sleeping in the main play area in this manner. Infant head was placed on an adult sized pillow and their entire body was covered with the blanket, including their entire head. Licensing asked the Licensee to uncover sleeping infant’s face and head. Importance of keeping infant’s head and face uncovered during sleep was discussed with the Licensee. After discussion Licensing was informed that this child was not enrolled in this facility but was enrolled in the other facility whose Licensee was present at this inspection. Licensing discussed using the sleeping equipment available at this facility for this child. Licensee of the other facility stated they they did not want to use the sleeping equipment available at this facility.

    Corrective action. Licensee must ensure that each piece of furniture or equipment for sleeping is appropriate for the size, age and development of the children intended to use it.

    A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.

    Observation. During inspection Licensing observed: -Wooden cover of the drawer under the sink in the washroom was unsecured and loose as a result of which it fell off. License attempted to fix it but it fell off again. -Electric cords of a printer, rice cooker and diaper wipe-warmer were unplugged and dangling loose and were accessible to children. Licensee made them all inaccessible during the inspection.(CORRECTED DURING INSPECTION) -Black wheels of the outdoor play tunnel were cracked and broken apart exposing the metal nails and metal parts inside the wheel. -Outdoor play kitchen had dried bird faeces on it. Licensing observed child playing with this equipment. -Outdoor play area had toys that were not used by children as they were not in good repair. Licensee is aware that some outdoor toys need to removed and outdoor resilient surfacing is to be replenished.This is a repeat contravention #LVIA-BLBNYJ

    Corrective action. Please ensure that healthy and safe environment is provided at all times while children are under the supervision of employees.

    Observation. Upon inspection of the backyard, there was a large gap under the gate that a child could go through.

    Corrective action. Please ensure that the perimeter of the yard is enclosed, so the children are free from harm.

    Observation. Upon inspection of the outdoor play equipment, there were a couple broken pieces of equipment with sharp corners, that could pose a hazard to the children. The play house outside was also coming apart which poses a fall hazard.

    Corrective action. Please ensure the outdoor equipment is free from hazards and is in good repair.

    Observation. Upon inspection there were 3 exposed outlets (powerbar) in the main play room.Licensee stated that she did not have a cover for the outlets and was trying to block them with the chair.

    Corrective action. Please ensure a healthy and safe environment is provided at all times.

    Observation. Upon review of the medication forms, there were 2 separate days where the dosage amount and administration time was not specified by the parent.
