    Observation. The diaper change table was observed in the nap room where there is no wash basin.

    Observation. In the play room there are electrical base board heaters that were hot to the touch. Entry to the rest of the home is accessible to children. There is a screw sticking out of a lower board at the bottom of the outdoor stairs near the sand box. The screws in the boards on the tires are not flush with the boards. The stumps are very slippery and close together creating a hazard.

    The safety zone around play equipment does not generally extend 6 feet in all directions (swings should extend a minimum of 6 feet from the outer edge of the support structure on each side and the front and back of the swing should extend out a minimum distance of twice the height of the swing as measured from the ground to the swing hangers on support structure).

    Observation. There are several pieces of play equipment within a small area and next to a porch post. There is not adequate space in between to provide a safe play zone for children to move in.
