  Observation. Licensing observed 3 chairs with straps in the bathroom. The manager explained that the staff strap the babies into the chairs while they are waiting for their diaper to be changed or waiting for another child to be changed. One staff will take all 4 of the babies into the bathroom at once.

  Observation. There is a child with a reported Anaphylaxis allergy. There is an outdated incomplete care plan on site. The care plan also includes the recommendation to use an antihistamine as a first response to an allergic reaction.
