Observation. Infant room has a fridge which is used to store bottles. There is no thermometer within the fridge in order to show whether the fridge temperature is functioning at 4 Degrees Celsius or below.
Corrective action. Manager stated that they would place a thermometer within the fridge so that the temperature of the fridge can be monitored.

Observation. Licensing observed the following: - outdoor play space for infant toddler program includes small rubber pieces, small enough to present a choking hazard, which are used as a resilient surface. The manager confirmed that children in this program put the rubber pieces in their mouths. - children's unwashed blankets stored in a pile which is not sanitary - an unsecured cord for a sound device in a naproom which poses a risk of entanglement
Corrective action. Ensure a healthy and safe enviornment is provided at all times.

Observation. Licensing observed the following: - visibly soiled areas on a wall and baseboards/flooring in an infant toddler room - a gab in the flooring between two rooms in an infant toddler room were debris is collecting - a small tear in a diaper change mat (the manager removed and replaced during the inspection)

A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance. Observation (CORRECTED DURING INSPECTION): The diaper change pad in room 2 had a tear between 1-2inches in the plastic material, making it unable to be properly sanitized. The Manager corrected this on site by replacing the diaper change pad.
Corrective action. Ensure that all furniture, equipment and fixtures are clean and in good repair.

If a person prepays part of the cost of services provided by a class of community care facilities designated by the Lieutenant Governor in Council, the licensee must, at the time of the prepayment, deliver to the person a written statement setting out the terms and conditions on which a refund of all or any of the prepayment will be made.
Observation. The Licensee did not provide a written statement outlining the terms and conditions of refund to a person at the time of prepayment. The Licensee contact created a prepayment policy during the inspection and will post the notice for all parents. He will create a document for parents to acknowledge that they've read it.
Corrective action. Ensure each person who prepays is given a written statement outlining the terms and conditions on which a refund will be made.

- EQUIPMENT AND FURNISHINGS
A licensee must ensure that the entire outdoor play area is (a) enclosed in a manner that is suitable for the age and development of children, and will ensure that children are free of harm.
Observation. The Outdoor play area south west corner gap between the fence and the wall has not been addressed by the facility. A conversation was had with the Manager to create a plan on how to make this area inaccessible as he waits for the landlord to address the concern.
Corrective action. Ensure that the outdoor play area is enclosed in a manner suitable for the age and development of the children. Provide an updated site plan with the measurements and fences for the outdoor play area.
11160 - CCLR s.16(3)(a) - EQUIPMENT AND FURNISHINGS
A licensee must supply equipment, furniture and supplies that are (b) located so as not to block or hamper an exit in the case of fire or other emergency.
Observation. It was observed that the fire exit door in the Group Child Care (30 months- school age) program is bolted shut with 2 sliding bolts. If needed a child would not be able to unlock the top bolt in case of an emergency. A photograph was taken and is available to the facility upon request.
Corrective action. Ensure that the fire exit is not blocked or hampered.
11090 - CCLR s.15(1)(b) - OR SUPERVISION
A licensee must ensure that children are supervised at all times by a person who is an educator, an assistant or a responsible adult.
Observation. At Licensing arrival a child from the Group Child care (30 months- school age) program was unsupervised, asleep on the futon in the front hallway area and all classroom doors were closed. When Licensing inquired with the staff in the program, 1 staff person was unaware that the child was there and anther staff person said that the child is always supervised by the Manager. The Manager was not on site when Licensing arrived.
Corrective action. Ensure that the children are supervised at all times.
14060 - CCLR s.39(1) - OR SUPERVISION
Without limiting subsection (1), a licensee must ensure that (b) the ratio of employees to children attending a community care facility is no less than that permitted in Schedule E.
Observation. It was observed at the arrival of the Licensing Officers that the facility had a total of 39 children and 7 staff present in 3 rooms with only 1 educator in the Group Child (under 36 months) with 6 children. There was 1 employee short on site during this time. The staff and Manager said it was only during nap time when staff take breaks. A conversation was had that the ratio of children to staff must be maintained on site. The Group Child Care (Under 36 months) there was no Infant toddler educator on site for both classrooms. A responsible adult is scheduled as the third staff in the Group child care (30 months-School Age) program.
Corrective action. Provide a staffing plan to the Licensing Officer including staff break coverage. Ensure that the ratio of children to staff is no less than permitted in Schedule E.

Observation. The following was observed: -paint chipping on the walls exposing drywall -the toilet in the infant classroom was not clean.

11010 - CCLR s.13(1) - EQUIPMENT AND FURNISHINGS
A licensee must ensure that the entire outdoor play area is (a) enclosed in a manner that is suitable for the age and development of children, and will ensure that children are free of harm.
Observation. On the south west corner of the playground there is a gap between the wall and the chain link fence about a foot wide. Currently there are logs, broken planters and brambles located in this gap. There is a potential for a child to climb between the wall and fence. This is not on the site plan.
Corrective action. Ensure that the play area is enclosed in a manner that is suitable to the age and development of the children. Please provide an updated site plan of the outdoor play area.

Observation. The couches at the front are visibly dirty. The Licensee said he will have them washed.

A licensee must establish a program to instruct children in, and to practise the rules of, health and hygiene.
Observation. Licensing observed the following: -The nap bedding in room 3 was touching each other. -Toys were located next to the toilets in the infant toddler room.
Corrective action. Ensure that bedding is not touching each other. Ensure that toys are sanitized and stored appropriately.

Observation. The following was observed in room 3: -Paint chipping on the walls (this is a repeat contravention), -mouse feces was observed under the boys bathroom sink, -ants were observed coming in from the door to the playground.
Corrective action. Ensure that a healthy and safe environment is provided. Please provide the written response of how you addressed the contraventions.

A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.
Observation. Floors, baseboards, doors, and shelves are visibly dirty. Room 3 bathrooms were observed dirty and in need of cleaning. The couches at the front of the facility are visibly dirty and the Manager confirmed that sometimes children use this area.
Corrective action. Ensure that the furniture, equipment and fixtures are clean and in good repair.
11020 - CCLR s.13(2) - EQUIPMENT AND FURNISHINGS
A licensee must have one toilet and wash basin for every 10 children or fewer.
Observation. The washroom in Room 1 was being used to store toys. This was identified at a previous routine inspection.
Corrective action. Ensure that the toilet is available for use.
11060 - CCLR s.14(2) - EQUIPMENT AND FURNISHINGS
A licensee must have at least 6m² of outdoor play area for each child.
Observation. The outdoor play area does not measure to have all children in care outside at the same time. The Manager provided a plan after the last routine inspection to ensure that children were not outside at the same time or crossing between programs. The facility was not following the plan today. It was observed today that 43 children from 2 different service types were outside at the same time.
Corrective action. Ensure that the plan put forward by the Manager is followed.
11140 - CCLR s.16(1)(a) - EQUIPMENT AND FURNISHINGS
A licensee must ensure that the entire outdoor play area is (a) enclosed in a manner that is suitable for the age and development of children, and will ensure that children are free of harm.

Observation. The nap bedding in room 3 was observed touching each other in the bags hanging and on the floor or nap beds on the floor. The Manager stated that it was set up from the day before.
Corrective action. Ensure that bedding for each child is not touching another child's bedding.
15030 - CCLR s.46(1) - LICENSING
A licensee must display in a prominent place in the community care facility both the licence, including any terms or conditions of the licence, and the name of the manager.
Observation. The name of the Manager is not posted.

10050 - CCLR s.11(1) - MEDICATION
If a licensee has agreed with a parent to give a child any medication prescribed by a medical practitioner or provided by the parent, the licensee and his or her employees must ensure that the medication is (b) readily accessible to employees.
Observation. A child requiring medication did not have it available.
Corrective action. Ensure that any medication agreed to be given to a child is readily available.
16020 - CCLR s.53(1)(b) - OR SUPERVISION
Without limiting subsection (1), a licensee must ensure that (b) the ratio of employees to children attending a community care facility is no less than that permitted in Schedule E.
Observation. The facility did not have an Infant Toddler Educator (ITE) in Room 2. The ITE for room 1 is not scheduled to work daily. This is a repeat contravention.
Corrective action. Ensure the ratio of employees to children is no less than schedule E. Please provide a staffing plan to the Licensing Officer.
14030 - CCLR s.34(2)(b) - RECORDS AND REPORTING
In respect of a record referred to in this Division, a licensee must (a) keep in a single place at the community care facility the records referred to in sections 56 to 58.
Observation. 2 care plans for children were unavailable for review.
Corrective action. Ensure that each care plan is kept in a single location in the facility.
19320 - CCLR s.59(a) - RECORDS AND REPORTING
A licensee must keep current records for each child showing (a) the information set out in subsection (2), (b) if applicable, the information and agreements set out in subsections (2.1) and (2.2), and (c) the consents referred to in subsection (3).
Observation. One child did not have an emergency contact.
Corrective action. Please ensure all children's records are current.

A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.
Observation. During physical inspection, Licensing observed the following: - A cupboard door under the diaper changing station in the Group Child Care(30 months to school age) bathroom falls off when opened. - The gate in the outdoor play space does not latch properly - There are weeds in the outdoor play space - The paint on many of the walls in the play spaces is chipping. This could be hazardous for young children if picking the paint and putting it into their mouth.
Corrective action. Please ensure that all areas are clean and in good repair when children are in attendance.
11020 - CCLR s.13(2) - EQUIPMENT AND FURNISHINGS
A licensee must ensure that children do not have access to any object or substance that may be hazardous to the health or safety of a child.
Observation. Licensing observed cleaning products in a cupboard under the sink in the bathroom located in the hallway. Although this bathroom is primarily used for staff, the door was open and the cupboard was accessible to children. Licensing did note that later in the inspection the door to the bathroom was closed.
Corrective action. Please ensure any hazardous material/substance is inaccessible to children at al times.
11200 - CCLR s.17 - LICENSING
Applicants for licences and licensees must notify a medical health officer immediately of any change in the information provided under section 9.
Observation. On review of the outdoor play space, Licensing observed that a large climbing structure had been added to the Group Child Care (under 36 month) program. In discussion with the staff it was stated that the play yards had been switched and this play structure was now in the Group Child Care (30 months to school age) play space. The Licensee had not consulted with Licensing as to this change in the information provided under Section 9. This change does not meet approval with Licensing as the children need to cross into each other's playspace on a daily basis when accessing through their program room doors.
Corrective action. Manager stated that the fence between the two play spaces could be removed to have one large play space with the groups sharing the space. Licensing will require a play space schedule and an updated site plan showing the changes to be submitted. Please provide a compliance plan as to when this change will take place.
10010 - CCLR s.10(1) - OR SUPERVISION
A licensee must ensure that children are supervised at all times by a person who is an educator, an assistant or a responsible adult.
Observation. During inspection, Licensing observed 1 child napping in a second room, separate from the program play space. The child was alone and loud music was playing in the nap space. No supervision aids were in place and all staff were working in the program space. When questioned how they would hear the child if requiring assistance, Staff indicated that the baby monitor was broken and so staff pop in to the nap room every 5 minutes to check on the child. During the inspection, Licensing observed that the child was not checked on for almost half an hour. There is concern that with the loud music playing and lack of constant supervision, a child could be in distress without a staff being aware.
Corrective action. Please ensure that children in a napping space away from the rest of the group are supervised appropriately.
14060 - CCLR s.39(1) - STAFFING
A licensee must ensure that each employee (c) practices implementing the emergency plan at least once each year.
Observation. On review of fire drill and evacuation records, Licensing observed that the annual evacuation drill had not been conducted since 2018. There are new staff employed at the facility who have yet to practice the evacuation drill.
Corrective action. Please ensure that all staff have knowledge of the procedure and practice the drill to evacuation, with the emergency supplies, to the secondary location.
12200 - CCLR s.22(2)(c) - STAFFING
A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (b) character references in respect of the person.
Observation. On review of 3 staff files, Licensing observed that 1 file was missing character references. Licesning notes that a list of referees was in the file but with no indication of whether these referees had been contacted to provide a reference for the staff.
Corrective action. Please ensure that staff are appropriately screened prior to hire.

A licensee must not employ a person in a community care facility as (a) an educator or an assistant unless the person holds a certificate issued under Division 2 [Employee Qualifications], which must be verified by the licensee using a system established and maintained by the director of the early childhood educator registry,
Observation. On review of 3 staff files, Licensing observed that 1 staff was missing a validation for their ECE certification. Licensing notes that all other validations were in place for certified staff.
Corrective action. Please ensure all areas of Section 19 are screened prior to employment.
