  A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.

  Observation. Licensing observed the following: - a portable heater in use within children's reach - a heavy object on open shelving with an unsecured electrical cord that was within the reach of children in a nap room; posing a risk if a child were to pull the cord and the heavy object could fall; the cord also poses a risk of entanglement - an uncovered electrical outlet in a nap room

  Corrective action. The heavy object on a shelf with an unsecured cord was removed during the inspection, along with the portable heater. The Licensee will ensure that all electrical outlets are covered.

• OR SUPERVISION

  Without limiting subsection (1), a licensee must ensure that (b) the ratio of employees to children attending a community care facility is no less than that permitted in Schedule E.

  Observation. Two Group - under 36 month classrooms did not have an IT staff in ratio during staff break, as required.

  Corrective action. The Licensee will revise the staff break schedule to ensure that Schedule E is maintained at all times.

  14030 - CCLR s.34(2)(b)
• STAFFING

  The licensee must not permit a person over the age of 12 to be ordinarily present on the premises of a community care facility while children are present, unless the person is of good character and the licensee has obtained a criminal record check for that person.

  Observation. A criminal record check for a person over the age of 12 who is ordinarily present was not obtained.

  Corrective action. Licensee will obtain a CRC from the criminal records review program for this individual.

• STAFFING

  A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (a) a criminal record check for the person.

  Observation. Four staff files were reviewed. One of the four CRC's obtained is not specific to the Licensee, as required.

  Corrective action. The Licensee will obtain a CRC for this staff.

• EQUIPMENT AND FURNISHINGS

  A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.

  Observation. Licensing observed the following: - There is insufficient shade provided to protect children from sunburn and sunstroke - A refrigerator that staff explained is for storing children's bottles is located in a bathroom - Diaper cream was stored accessible to children, under a sink in the children's bathroom. This was removed by a staff during the inspection.

  Corrective action. A staff said that shade will be provided today and the refrigerator will be moved out of the bathroom.

• HYGIENE AND COMMUNICABLE DISEASE

  A licensee must establish a program to instruct children in, and to practise the rules of, health and hygiene.

  Observation. Licensing observed staff applying sunscreen to multiple children; without washing their hands between each child/application which is not hygienic as this could spread illness.

  Corrective action. A staff confirmed that staff will wash hands in between each application of sunscreen.

• POLICIES AND PROCEDURES

  A licensee must: (b) have an emergency plan that sets out procedures to prepare for, mitigate, respond to and recover from any emergency.

  Observation. Emergency Supplies are insufficient to implement the emergency plan

  Corrective action. A staff confirmed that sufficient emergency supplies to care for children for 72 hours will be obtained.

  13020 - CCLR s.22(1)(b)
• STAFFING

  A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (b) character references in respect of the person.

  Observation. Licensing reviewed three staff files and one did not include references. Additionally, Licensing reviewed two student files and neither included references.

  Corrective action. A staff confirmed that references will be obtained.

• EQUIPMENT AND FURNISHINGS

  A licensee must ensure that children do not have access to any object or substance that may be hazardous to the health or safety of a child. Observation (CORRECTED DURING INSPECTION): Licensing observed a lysol cleaner in the bathroom cupboard that had no lock. Staff locked the cupboard once addressed.

  Corrective action. Ensure that children have no access to any object or substance that may be hazardous to the health or safety of a child.

  11200 - CCLR s.17

  Without limiting subsection (1), a licensee must ensure that (b) the ratio of employees to children attending a community care facility is no less than that permitted in Schedule E.

  Observation. Licensing reviewed the staff qualifications in 2 classrooms and determined that they did not meet the requirements as in schedule E.

  Corrective action. Ensure that ratios are met according to schedule E.

• POLICIES AND PROCEDURES

  A licensee must: (b) have an emergency plan that sets out procedures to prepare for, mitigate, respond to and recover from any emergency.

  Observation. Licensing reviewed the emergency kits for each classroom, 7 out of 8 kits have expired food. The manager said that staff were told to go through them in December and let families know.

  Corrective action. Ensure that all emergency kits have supplies, food, and water that is not expired.

  13020 - CCLR s.22(1)(b)
• RECORDS AND REPORTING

  A licensee must keep, for each child, a record showing the following information: (d) name and telephone number of a parent, medical practitioner and emergency contact.

  Observation. Licensing reviewed 14 children's files; observed 1 child is missing their medical practitioner's name and telephone number.

  Corrective action. Please ensure children's files are complete, a medical walk in clinic is acceptable.

• RECORDS AND REPORTING

  A licensee must keep, for each child, a record showing the following information: (a) name, sex, date of birth, medical insurance plan number and immunization status.

  Observation. Licensing reviewed 14 children's files; 3 missing their immunization status.

  Corrective action. Licensee must ensure that for each child, there is a record showing the child's immunization status. Please ensure there is a system in place to review children's files.

• STAFFING

  A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (f) evidence that the person has complied with the Province's immunization and tuberculosis control programs. Observation (CORRECTED DURING INSPECTION): Licensing reviewed 8 staff files; 1 missing their immunization status.

  Corrective action. Please ensure that staff files are complete and immunization status is obtained.

  12090 - CCLR s.19(1)(f)

• Without limiting subsection (1), a licensee must ensure that (a) children attending a community care facility are divided into groups, with each group having no more children than that permitted in Schedule E.

  Observation. Licensing confirmed there are 3 children under 3 years of age in attendance in the 30 months to School Age program.

  Corrective action. A licensee must ensure that children attending a community care facility, are divided into groups, as per schedule E

• A licensee must ensure that food and drink are not used as a form of reward or punishment for children.

  Observation. Licensing determined that food is used as a form of reward and is part of a toilet training strategy.

  Corrective action. Ensure that food and drink is not used as a form of reward or punishment for children.

• A licensee must ensure that the policies and procedures referred to in subsection (1) are implemented by employees.

  Observation. Licensing observed that written policies respecting food and drink exist but are not implemented by employees.

  Corrective action. Ensure that the policies and procedures referred to in subsection (1) are implemented by employees.

• EQUIPMENT AND FURNISHINGS

  A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.

  Observation. A large metal play structure in the children's outdoor space has a broken section with a jagged rusty edge. The manager explained that the facility is awaiting repair of the equipment and during the inspection covered the jagged rusty edge with heavy tape.

  Corrective action. Ensure that the equipment is in good repair.

• MEDICATION

  If a licensee has agreed with a parent to give a child any medication prescribed by a medical practitioner or provided by the parent, the licensee and his or her employees must ensure that the medication is (a) administered to the child in the amount and at the times specified by the child's parent or in the child's record or care plan.

  Observation. There were unclear written directions specified by the parent regarding the administration of medication.

  Corrective action. Ensure to obtain written directions by the parent regarding administration of medication.

• POLICIES AND PROCEDURES

  A licensee must keep current records of each of the following: (c) a record respecting compliance with section 22 (2) (b) and (c) [emergency training and equipment]. Observation (CORRECTED DURING INSPECTION): Licensee confirmed that fire drills are conducted monthly; however, a record has not been maintained.

  Corrective action. A record of fire driills that have been conducted was created during the inspection.

  A licensee must keep, for each child requiring extra support, a current care plan showing the following information: (a) the diagnoses relevant to the child's requirement for extra support, as made by health care professionals.

  Observation. No care plan available for a child requiring extra support.

  Corrective action. Ensure to obtain a care plan for each child requiring extra support.

• STAFFING

  A licensee must ensure that each employee (c) practices implementing the emergency plan at least once each year.

  Observation. Licensee explained that employees do not practice the emergency plan annually.

  Corrective action. Ensure to practice the emergency plan at least once each year.

• STAFFING

  A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (f) evidence that the person has complied with the Province's immunization and tuberculosis control programs. Observation (CORRECTED DURING INSPECTION): Of the five staff files reviewed, two did not include immunization status.

  Corrective action. The manager obtained immunization status during the inspection.

  Observation. The manager confirmed that food was given to children and this information was not made available to parents.

• A licensee must (a) ensure that behavioural guidance is appropriate to the age and development of the child who is receiving the guidance.

  Observation. Licensing determined that a staff has yelled at children.

  14220 - CCLR s.51(1)(a)

• RECORDS AND REPORTING

  A licensee must notify the medical health officer within 24 hours after (a) a child is involved in, or may have been involved in, a reportable incident described in Schedule H while under the care or supervision of the licensee.

  Observation. Licensee did not notify the licensing officer within 24 hours of an incident as described in Schedule H as required. A group child care (under 36 month) program was voluntarily closed due to Covid-19 cases.

  Corrective action. Please ensure this is reported to Licensing as prescribed.

  A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.

  Observation. LO observed and discussed the following with the manager: - 1 toddler classroom uses designated cotton towels per child, per day. If cotton towels are used, please ensure that single-use towels are available after each hand washing. - cleaning supply was accessible to children in PS/OSC room ( under the washroom sink).

  Corrective action. Please ensure that a healthy and safe environment is provided while children are in care.

  Observation. The name of the manager was not posted onsite.

  Corrective action. Please ensure that manager's name is posted in a prominent place within the facility.

• EQUIPMENT AND FURNISHINGS

  A licensee must ensure that children do not have access to any object or substance that may be hazardous to the health or safety of a child.

  Observation. Licensing observed the following: - Plastic bags stored in cupboards under the sinks in both the Preschool 2 room and the Infant/Toddler #1 bathroom. Licensing notes that the bathroom door has a child lock on it, but once inside, the cupboard is accessible to children. - The laundry room door was open leaving hazardous items such as bleach, laundry detergent accessible to children.

  Corrective action. The hazardous items in the laundry room had been removed on the second day of inspection but the door was still open. Please ensure the room is inaccessible to children at all times and that a safe environment is provided.

• LICENSING

  A licensee must display in a prominent place in the community care facility the certificate, or a copy of the certificate, of any educator or assistant employed to work in the community care facility.

  Observation. Licensing observed that 2 staff certificates were not posted in the facility. Discussed with Manager tha tone staff is a break coverage staff and not sure where the certificate should be posted. Licensing provided recommendations.

  Corrective action. Please ensure that all qualified staff certificates are posted in the facility.

• RECORDS AND REPORTING

  A licensee must keep, for each child, a record showing the following information: (i) a photograph or digital image of the child, and other information that can be used to readily identify the child in an emergency.

  Observation. Licensing observed that a random selection of children's files did not contain photographs. Supervisor stated that this was a work in progress.

  Corrective action. Please ensure that digital images/photos are available with emergency information for ease of identifying in an emergency.

  Observation. Licensing observed 3 missing immunization status out of 6 children's files reviewed. Supervisor did say that some were missing prior to review. Discussed the requirement of Section 57 (2)(a)

  A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (a) a criminal record check for the person.

  Observation. Licensing reviewed 5 staff files and observed that 2 staff did not have site specific criminal record checks on file. They did, however, have current clearances from a previous facility. Supervisor said that they had been sent for but there is no evidence to speak to the request for sharing being made.

  Corrective action. Please ensure that criminal record checks are in place for the work site prior to employment.

  A licensee must ensure that each employee (b) practices implementing the fire drill system at least once each month.

  Observation. Licensing observed that fire drills had not been practiced and recorded since the opening of the facility in May 2020 apart from the month of July.

  Corrective action. Please ensure that fire drills are practiced on a monthly basis and recorded appropriately.

  Observation. Licensing reviewed 5 staff files and observed that 1 staff file did not contain a record of reference checking for this staff. Discussed with the Supervisor as to the requirements for staff screening.
