    Observation. C4.1E - Reg. 57 - All child records did not include a photo and other identifying information that can be used to readily identify the child in an emergency. The Licensee had digital photos of the children on a cell phone however there was no system implemented on the digital image to identify the child name. Upon discussion with the Licensee a system will be implemented with printed child photos.

  • C2.1 - Are written policies and procedures in place to guide employees in all matters regarding the care and supervision of children?Non-critical

    Observation. C2.1B - Reg. 22 - There is an emergency evacuation plan in place however it is incomplete of potential emergency considerations for lock down, earthquake, and evacuation procedures do not identify neighborhood or community relocation sites or means of transportation. The system currently in place does not ensure there is adequate supplies if leaving the property. The plan should be thorough of how to prepare, mitigate, respond and recover from any emergency. Additional information has been provided with this report regarding emergency planning considerations. C2.1F - Reg. 56 - There was no record available indicating that the Licensee had practiced implementing the emergency plan annually.

  • C3.1 - Does the facility have sufficient employees who possess adequate training and experience to meet the care, supervision, and needs of the children?Non-critical

    Observation. C3.1H - Reg. 20 - A person over the age of 12 periodically on the premise during operating hours does not have a criminal record check. A resource of the criminal record check application process was provided with this report.

    Observation. Ensure records are kept of notification parents receive with regard to food and drink provided to children.

    Observation. Ensure all climbing equipment is installed as per the manufacturers installation and safety guidelines. It is noted that resilient material is not adequate at this time.

    Observation. The emergency plan has not been practiced at this time. Ensure it is practiced at least once each year.

  • C4.1 - Are facility records current and complete?Non-critical

    Observation. Some child records are missing start dates, emergency contact names and numbers, doctors phone numbers. You stated you have photos of all children in your phone. As discussed keep a log of all play space inspections, maintenance and repairs in a format that works for you.

  • C6.1 - Are acceptable hygiene practices maintained?Non-critical

    Observation. Today it was observed that the children use an outdoor potty which is not recommended. Hand washing did not occur after use. It is best practice to use the inside toilet with the children to ensure health, hygiene, and privacy (for those who may need this). Please submit a toileting plan in this regard. The change table has a cloth cover that cannot be sanitized after each use. You stated you have a clean one-time use cloth for each child. Please include this in your toileting policy.
