Observation. Licensing was not notified that a new Manager was hired for the facility in November 2025. It is required that Licensing is notified whenever there is a change of facility Manager. Submit to Licensing by December 23, 2025 a change of Manager notification.

Observation. A staff file is missing a record of their immunization status. Staff files must be accurate and complete prior to starting employment. Corrected during inspection.

Observation. A child's care plan does not indicate if its been reviewed or developed with the parent. Care plans must be developed and reviewed annually with the parent to provide transparency. Submit to Licensing by January 6, 2026 evidence the care plan has been reviewed with the parent.

Observation. A broken art easel in the preschool outdoor play area is accessible to children. Broken equipment provides an unsafe environment to the children in care. Submit to Licensing by December 18, 2025 confirmation the broken equipment has been removed.

Observation. Child records were reviewed, one child's records were missing the name and phone number of a medical practitioner. The attendance records used for the Preschool program do not record the time the children are sign out of the preschool program program and into the Group Child Care 30 months to School Age. The record for the outdoor play space checks are not consistently being completed.

Observation. The natural logs in the outdoor play space were very slippery and pose a potential risk to children.

Observation. A staff with an expired ECE License to practice was observed working in Group Child care (30 Months to School Age) program in an ECE capacity. Five staff ECE/A certificates have not been verified.

Observation. It was observed a medication located in the facilities medication storage box has expired.
