  Observation. It was alleged that during three incidents a child was injured without notification to the parent. In review of the incident log book, employee communication book and in speaking with employees, it has been determined there is inconsistent notification to the parents and incomplete log book entries of minor accidents. Allegations to Section 55 (1)(a) Notification of illness or injury and Section 56 (1)(f) Community care facility records and policies are found: Substantiated. Provide to licensing, by September 2, 2025, a detailed written plan which addresses how you have corrected these contraventions.

  Observation. The Licensee’s system for ensuring that employee files contain all of the legislated requirements was noted to be ineffective. Licensing found staff files to be incomplete. Missing items included: Immunization status, copies of certification, work history, references and a Criminal Record Check.

  Observation. Some children's files were missing photos of the child and emergency contact information.

  Observation. Observations: System in place to ensure children's records are complete. LLO tested system and found child files to be complete, log of minor injuries/incidents unexpected events updated regularly, reportable incidents reported and mitigated appropriately. Contravention: The LO reviewed the attendance binder. Occasionally departure times were missing.

• C7.1 - Is the facility safe, sanitary, free from hazards, in good repair, and appropriate for the age and development of children?Non-critical

  Observation. Observations: The majority of the facility appears to be safe and free from hazards fridge temperature below 4° C. Contravention: Hot water was measured at 60 C today
