Observation. - Some staff files were missing required information such as detailed character reference checks, a resume, and immunization status. A person must not be employed unless the information available indicates the current status of their immunizations, that the person is of good character, and that the person has relevant work experience. *This is a recurring contravention as it was also noted on the last routine inspection report.* *Correct by: December 1, 2025.* - Some children who were present and attending the Group Child Care (School Age) program were over 13 years of age. As per Section 1 of the Community Care and Assisted Living Act (CCALA), a "child" is defined as "a person under the age of 13 years". Once a child turns 13 years old, they are no longer eligible to be under the care of the Licensee. Any child who is over the age of 13, and who is ordinarily present in the facility, must have a valid criminal record check. *Correct by: November 25, 2025.*
- C4.1 - Are facility records current and complete?Non-critical
Observation. - Some of the personal information in the children's records, such as doctor/clinic contact information and emergency contacts other than the parents, was missing. To ensure that the staff have immediate access to the children's personal information in the event of an emergency situation and/or evacuation, this information must be complete and up to date. *This is a recurring contravention as it was also noted on the last routine inspection report.* *Correct by: December 1, 2025.* - The daily attendance records were missing information such as the correct arrival and departure times and whether or not a child was absent. Accurate attendance records allows the program staff to quickly account for every child during an emergency situation and provides a clear record of who is present in the facility at any given time. *This is a recurring contravention as it was also noted on the last routine inspection report.* *Correct by: November 25, 2025.* - Some staff records could not be reviewed as they were not readily available. The required documents for staff must be complete, up to date, and kept on site at the facility. The staff documents obtained by the Licensee will ensure that the person is of good character, has the personality, ability, and temperament necessary to work with children, and has the training, experience, and demonstrates the skills necessary to carry out the duties assigned to them. Having staff records on site and readily available will also assist public health officials and/or emergency personnel in the event of a disease outbreak and/or emergency situation. *This is a recurring contravention as it was also noted on the last routine inspection report.* *Correct by: December 1, 2025.* The play space inspection log was up to date and appeared satisfactory. The system for recording minor incidents was in place and appeared satisfactory. Photos of the children in care were included with the children's emergency records.

Observation. - Hazardous products kept under the kitchen sink were accessible to the children in care. To prevent potential injury to the children, hazardous products must be kept inaccessible. *This is a recurring contravention as it was also noted on the last routine inspection report.* *Correct by: November 25, 2025.* - The thermometer in place for displaying the fridge temperature was broken. To ensure safe food storage and prevent food borne illness, the thermometer must be in good working order and show that the fridge temperature does not exceed 4°C. *This is a recurring contravention as it was also noted on the last routine inspection report.* *Correct by: December 1, 2025.* Rooms deemed off limits to the children in care were locked.

Observation. - During the inspection, the Licensing Officer (LO) observed that the name of the manager was not posted in the facility as required. *After the inspection, this contravention was confirmed as resolved by Licensing on Dec 11/24.
- C3.2 - Are persons qualified and appropriately replaced?Critical
Observation. - During the inspection, no records pertaining to the required training for the 2 program staff were available to be reviewed by the Licensing Officer. As such, the LO could not verify if the staff persons employed in the program have the training and work experience to qualify as responsible adults. *After the inspection, the LO received a copy of the manager's relevant training certificate on Dec 11/24.
- C10.2 - Are children supervised, and are employee to children ratios maintained?Critical
Observation. - The LO observed 2 children in care at the start of the inspection. 4 additional children arrived at later times during the inspection. No records pertaining to the required training for the staff (manager) present was available to be reviewed by the Licensing Officer. As such, the LO could not confirm if the manager had the training and work experience in place to qualify as a responsible adult and, therefore, the staff to child ratio could not be verified as being met. *After the inspection, the LO received a copy of the manager's relevant training certificate on Dec 11/24.
- C2.1 - Are written policies and procedures in place to guide employees in all matters regarding the care and supervision of children?Non-critical
Observation. - During the inspection, the LO observed that the fire drill system was not displayed as required. - During the inspection, the LO observed that monthly fire drills were not recorded.

Observation. During the inspection, no records pertaining to the following were available to be reviewed by the LO: - Child records as per what is required in Section 57 of the Child Care Licensing Regulation. - Minor incident log. - Written permission from the parents to call for an ambulance for their child when needed. - Emergency contacts listed for the child other than the parents. - Current photos for each child that can be used to readily identify the child in the event of an emergency. - Any allergies of the children in care. *After the inspection, this was confirmed as resolved by Licensing on Dec 11/24. During the inspection, incomplete records pertaining to the following were reviewed by the LO: - Some of the daily attendance records were missing arrival and departure times and when a child was absent. - The outside play space inspection log was not up to date. The LO and the manager discussed the requirements specific to ensuring that the record keeping systems in place are kept up to date and are readily available for review on site at the facility. The LO and the manager discussed the requirements and the importance of keeping complete and readily available child records, including daily attendance and photos of each child, in the event of an emergency or evacuation.

Observation. - During the inspection, the LO observed hazardous cleaning products accessible to the children in care. - During the inspection, the LO observed the furnace room unlocked and accessible to the children in care. *After the inspection, this contravention was confirmed as resolved by Licensing on Dec 11/24. - A system for measuring the temperature of the fridge was not in place during the inspection. To ensure safe food storage and prevent food borne illness, the fridge temperature must not exceed 4°C.
- C3.1 - Does the facility have sufficient employees who possess adequate training and experience to meet the care, supervision, and needs of the children?Non-critical
Observation. - The criminal record check required for the "art teacher", who regularly visits the program to teach art to the children in care, was not available for the LO to review on site at the facility during the inspection. *After the inspection, this contravention was confirmed as resolved by Licensing on Dec 11/24. - Other than the criminal record checks for the 2 program staff employed by the Licensee (including the manager), no other staff records were available for the LO to review during the inspection. The other records required in a staff file but were not available for review include: a resume, evidence of relevant training, immunization status, evidence of at least 2 character reference checks, and a valid first aid certificate (if applicable). The criminal record checks for the 2 staff were reviewed by the LO and were valid.
- C5.1 - Are healthy eating and nutritional habits promoted and practiced?Non-critical
Observation. - Parents provide food for their child, however, in the event that a child does not have any food for an afternoon snack, the Licensee will provide popcorn and fruit. When asked by the LO, the manager was not aware that a system needed to be in place to inform parents if any food was given to their child. The LO observed that safe drinking water was readily available for the children.
