    Observation. The complaint alleges that a domestic disturbance occurred at the facility while children were in care. Staff provides an account of the incident, including timelines, and confirms that children were present at the time. A review of attendance records further verifies that some children were in care during the alleged incident. Based on the information received, the allegation is substantiated. Submit to Licensing by June 24, 2026, a report regarding the incident that has occurred on June 17, 2026, along with a plan to ensure that this will not occur again.

    Observation. At the time of inspection, the children's arrival times were not recorded. Ensure to keep a daily attendance record indicating whether each child is absent or, if the child is present, the time of arrival and departure.

    Observation. The sandbox cover requires repair. Additionally, a playpen in the indoor space needs to be secured. Ensure the facility, equipment and fixtures are clean and in good repair. The Licensing Officer observed a dome climber in the outdoor play space that requires protective surfacing. Moreover, the indoor play area has a climber and trampoline that require additional protective surfacing. Ensure protective surfacing is installed to reduce the risk of injury from falls.

    Observation. The fire extinguisher was due for servicing. The licensee must ensure that the fire extinguisher is working to prepare for, mitigate, respond to and recover from any emergency.

    Observation. • The diaper mat is in poor repair. • Some of the toys and storage bins in the outdoor play space are broken. • Some items in the gross motor room pose a potential hazard and need to be made inaccessible to children. • Some electrical cables in the main activity room pose a potential hazard and need to be secured.

  • C1.1 - Does the licensee continually inform licensing of any significant changes to the structure or operation of the facility?Critical

    Observation. Changes to the facility have occurred which may be a positive addition however licensing was not notified. Please provide updated site plans if you decide to continue to use the back half of the yard. This is an added space. Also provide updated floor plans as you have added the use of an additional room for the children.

  • C7.1 - Is the facility safe, sanitary, free from hazards, in good repair, and appropriate for the age and development of children?Non-critical

    Observation. The children have access to unsafe cleaning products and items stored in cupboards in the kitchen and bathroom upstairs. Child safety latches are not being used. The new added outdoor space has a section of fencing that is in disrepair and has fallen down. The licensee has made this added area inaccessible until a solution is found. The facility has added a Little Tikes climber to the outdoor playspace and to the inside space. There is no resilient material underneath or around these climbers presenting opportunity for injury. The Director of Licensing Standard of Practice - Safe Play Space states that any equipment must meet the manufacturers installation and safety guidelines. It is the responsibility of the licensee to ensure safety, installation and use of equipment at all times.

    Observation. Handwashing was not observed today for the children after diaper changes. Also ensure the change mat is sanitized after each use. The care provider stated this usually occurs however was missed today in the business of the inspection.

  • C7.1 - Is the facility safe, sanitary, free from hazards, in good repair, and appropriate for the age and development of children?Non-critical

    Observation. There were some electrical outlets upstairs that did not have child safety plugs, a glass mirror is broken but accessible to children (if upstairs), a broken vent with sharp edges needs repair in the mud/entry room and an electrical cord needs to be secured in the nap room.

    Observation. Some child records were found to be partially complete today missing doctor name and phone number, care card number, and emergency contacts.
