Observation. Upon review of employee records it was observed that one record required immunization status.

Observation. Upon review of a sampling of child records it was observed that one record required a doctor's name and number. In one program it was noted that when a child was visiting the time of arrival and departure from the program was not noted. Upon review of documentation one medication administration form did not include the dosage assigned by the medical professional.
