    Observation. Unrelated to the complaint allegation, in conversation with the facility manager, it was determined that the facility’s policy of operating using a system of “Primary Groups”, where certain staff care for certain children, restricted children’s ability to engage in certain activities based on their assigned staff member within the same program room. Additionally, children who were familiar with specific staff members were unable to engage with them due to these primary group assignments. This resulted in distress for some children, as all children and staff were present within the same program room. The primary group practice extends into toileting routines. When a child requires a diaper change, staff bring the three other children assigned to them into the washroom, despite the washroom being attached to the program room and having clear lines of sight, as well as support from additional staff members present in the room. The "Primary Group" practice unnecessarily disrupts engagement, peer interactions and introduces avoidable hygiene risks for children. Submit by June 24, 2026, how you have address this requirement.

    Observation. The complaint information received alleged a staff member struck a child with their hand and lifted children up by grabbing the backs of their jackets. Follow-up to this complaint included staff interviews, on-site observations, review of facility documentation (communication book and minor log book) and lack of evidence. As a result of the investigation it is determined that the complaint is unable to be substantiated. The complaint information received alleged a staff member called children "fat" and "bad" because they did not listen. Follow-up to this complaint included staff interviews, on-site observations and review of facility documentation (communication book, minor log book, manager notes). As a result of the investigation it is determined that the complaint is substantiated. The employee is no longer working at the facility and the risk to children has been mitigated. No further follow up is required at this time.
