• C10.2 - Are children supervised, and are employee to children ratios maintained?Critical

  Observation. It was alleged that staff members stated they had been out of ratio recently and that it was not the first time. Follow up to this complaint included a site visit to the facility, review of attendance records and information gathered from staff and the licensee contact. As a result of statements made during staff interviews and review of attendance records it has been determined that the facility was out of ratio for a short period October 21 2025 due to unexpected staff shortage on that day. Therefore, this allegation is substantiated. Findings indicated the staff on site corrected the situation within a short time frame. Submit a compliance plan to address this issue on or before November 7, 2025.

• C3.1 - Does the facility have sufficient employees who possess adequate training and experience to meet the care, supervision, and needs of the children?Non-critical

  Observation. It was alleged that children from multiple care program types are being combined in the mornings for longer than the 30 minute time period allowed by legislation. Follow up to this complaint included a site inspection at the facility, review of attendance records and information gathered from staff and the licensee contact. As a result of statements made during staff interviews and observation that two groups were still combined longer than 30 minutes during on site inspection, it has been determined that this allegation is substantiated. Submit a compliance plan to address this issue on or before November 7, 2025.

  Observation. 1 electrical outlet uncovered. Corrected during inspection.

• C3.1 - Does the facility have sufficient employees who possess adequate training and experience to meet the care, supervision, and needs of the children?Non-critical

  Observation. 3 employees had criminal record checks that were not processed through the licensee, but by other organizations. 3 employee files were missing ECE certificate verifications through the ECE Registry website.

  Observation. It was alleged that an employee at the facility made statements to other employees, encouraging them to not follow strategies in a child's care plan during an observational visit with an outside support and resource agency, in order to receive ongoing support for the child. During the investigation licensing conducted a site visit, employee interviews and reviewed facility documentation. As a result of the investigation it was determined that this allegation is substantiated.

  Observation. Educators and assistants have certificates posted and on file, but they have not been verified.

  Observation. Upon interview of the Licensee it was determined that one child was not provided continuous supervision, as they were not picked up from school. The Licensee has provided a plan to reduce risk of recurrence of this incident.

• C4.1 - Are facility records current and complete?Non-critical

  Observation. A child was involved in a Missing/Wandering incident, which was not reported within 24 hours. The incident report was submitted upon notification to the Licensee that this was required.

• C2.1 - Are written policies and procedures in place to guide employees in all matters regarding the care and supervision of children?Non-critical

  Observation. The facility has policies and procedures in place to guide employees in all matters regarding care and supervision of children. The Licensee recently updated and clarified their pick-up process in response to this complaint.
