  Observation. It was alleged that while a child was napping, a staff member pulled the blanket off the child and, after a few moments and without saying anything, lifted one side of the plastic sleeping cot, tilting it until the child rolled onto the floor. This action woke the child, who appeared dazed and confused. Reviewed the submitted reportable incident, which confirmed the allegation. Based on a review of the submitted incident report and discussions with the executive director during the investigation, the allegation is substantiated. The employee has been since terminated from the facility.

• C6.1 - Are acceptable hygiene practices maintained?Non-critical

  Observation. During the inspection, I observed that a couple of tables the children were about to eat on were visibly dirty. In addition, the children's extra clothing was being stored in cloth bags hung directly beside the toilet. Dirty eating surfaces and storing clothing beside a toilet can expose children to contaminants that may contribute to the spread of illness or communicable diseases. Submit by December 3, 2025, how you have corrected these hygiene practices.

  Observation. The complaint information received alleged a staff member was holding down a child's legs during nap time without a care plan. Based on the information gathered during the investigation which includes facility documentation, staff interviews and discussions with the facility manager the allegation is substantiated.
