• C7.1 - Is the facility safe, sanitary, free from hazards, in good repair, and appropriate for the age and development of children?Non-critical

  Observation. Upon inspection it was observed that sleep mats in the 30 Months to School Age program were stored with sheets on, in contact with other mats. In order to reduce risk of spread of illness, ensure that there is no contact between used beddings. The Licensee Contact had a staff person remove sheets from mats, all mats will be sanitized prior to use, and bedding will be stored in individual baskets between uses.

  Observation. Upon review of a sampling of care plans it was noted that one had no indication that it had been updated or reviewed with parents in the past 12 months, and two did not have creation dates noted, so it is unclear when these plans need to be reviewed. Also, parents had not signed the care plans, although it was noted that they had been notified of the plan.

  Observation. Upon review of a sampling of child records it was noted that one required a care plan in order to be complete. All other child record required information appeared to be present during review. Upon review it was determined that the Outdoor inspection log required further detail in order to be complete.

  Observation. Upon inspection of the outside play area it was noted that the sandbox tarp cover is deteriorated and has holes in it, allowing for unhealthy materials to get into the sand box. Upon discussion it was noted that the sandbox is not currently in use, and that new sand is on-order. Once received, the tarp will also be replaced to reduce risk of contamination. Also it was noted that the large play structure with slide is not 6 ft from the fence. Licensee to readjust the location of the structure to be in compliance.

• C3.1 - Does the facility have sufficient employees who possess adequate training and experience to meet the care, supervision, and needs of the children?Non-critical

  Observation. Upon review of a sampling of employee records it was noted that one required a resume and some required information regarding status of COVID-19 immunization. One validation for a qualification certificate was not present in the file. Criminal Record Check letters, First Aide certificates and qualification certificates were all noted to be present and not expired at this time.

• C3.1 - Does the facility have sufficient employees who possess adequate training and experience to meet the care, supervision, and needs of the children?Non-critical

  Observation. Upon review of all employee records it was noted that two require evidence of reference checks, one requires a valid criminal record check and one immunization record requires COVID-19 immunization status.

• C4.1 - Are facility records current and complete?Non-critical

  Observation. Upon review of a sampling of child records it was noted that one requires the child's gender to be noted, and one requires a care plan for an identified condition. Care plans must be developed with the parents and any supports they deem appropriate, and must be reviewed annually. Also, it was noted that an outdoor space inspection log is not in use at the facility. Discussed options for ensuring that daily inspections are completed and logged.

  Observation. It was noted that a new play structure in the outside play area needs to be moved approximately 1 foot, in order to be a minimum of 6 feet from the fence.
