Observation. The emergency plans do not indicate what equipment will be needed for an emergency evacuation. Emergency plans must indicate what equipment will be used. Submit to licensing by November 20, 2025, a written plan and evidence of the emergency equipment that will be used.

It was observed that two support therapists who are regularly scheduled to support certain children at the facility did not have evidence criminal record clearance or character references available. Some staff files are missing evidence of certificate validation, immunization, character references, criminal record clearances.

Observation. Radon results have not been received to licensing. A radon test kit has been deployed in the facility, and the results will be submitted to licensing upon completion of the long term test.
