  Observation. Some of the wooden outdoor play equipment is splintering and creating sharp edges that pose a risk to young children. Please ensure all outdoor wooden structures exposed to the weather are maintained so they do not pose a risk to children. An acceptable long term radon test result remains outstanding.

  Observation. Facility received unacceptable results for their radon test but have not mitigated to reduce the risk to children. Please provide licensing with a plan how you will mitigate.

  Observation. One child's record does not have their immunization status on file.

• Is the facility’s radon gas level in compliance with the 200 Bq/m3 limit ?Non-critical

  Observation. Test results indicate facility is not within compliance of the 200 Bq/m3 limit. Please provide a written plan by this date what steps you will take towards mitigation.

  Observation. Manager's file is missing their resume. One staff person does not have a criminal record check on file though an application for it has been completed.

  Observation. Verification of qualifications not found in the staff files. Manager began to correct during the inspection. Manager's file on site was found to be incomplete as most of her information is kept at the main office. Please ensure that a complete file as per Section 19 is kept on site for the manager.
