  Observation. The Licensing Officer observed two toilets for a program which is licenced for 24 children. The requirement is one toilet for every 10 children. The Licensee is required to submit a plan to come into compliance by the follow up date noted.

  Observation. It was observed that a child sized green sofa in the 30mos-School Age program is worn and with tears that cannot adequately be sanitized, which risks it harboring bacteria and germs. It was observed that there is no evidence of monthly or annual play space inspections being completed or recorded.

  Observation. Diaper pad found to be taped in several places due to tears. Please replace the diaper pad. Outside concrete ramp has deteriorated and poses as a tripping risk for children. Please ensure this ramp no longer poses as a risk to children.

• C3.1 - Does the facility have sufficient employees who possess adequate training and experience to meet the care, supervision, and needs of the children?Non-critical

  Observation. Staff files are missing verification of qualified certificates. One staff file is missing character references and immunization status.

• C4.1 - Are facility records current and complete?Non-critical

  Observation. Some children's files were missing information. Please ensure each child's record is complete. It was observed that the children's attendance records were missing some departure times. Please ensure attendance records are complete.
