Observation. Children did not wash their hands prior to eating their afternoon snack. Good hand hygiene is a crucial step in the prevention of infections and is the single most effective way to reduce the transmission of infectious diseases. Submit by December 09, 2025, confirmation that children are engaged in good hand hygiene practices.

Observation. The hot water was tested and found to be 54.4 degrees Celsius. Water over 49 degrees Celsius poses a scalding risk to children. Submit by December 05, 2025, confirmation that hot water accessible to children is 49 degrees Celsius or less.

Observation. The fire drill practice log was reviewed. There was no evidence that the fire drill was practiced in the month of September. The Licensee must ensure all employees practice the fire drill at least monthly. Submit by October 09, 2025, written confirmation the fire drill will be practiced monthly.

Observation. Child records were reviewed. Two records were found to be missing required information such as the child’s medical insurance number and immunization status. The Licensee is required to have current records for each child. Submit by October 09, 2025, written confirmation that the missing information has been added to each child's record. Attendance records were reviewed. Some absences were not recorded. The Licensee is required to keep attendance records including if a child is absent. Submit by October 09, 2025, written confirmation that all absences are being recorded. The play space inspection logs were reviewed. There was no evidence that a play space check had been conducted since mid September. The Licensee is required to conduct a thorough play space check daily and keep a log of this check. Submit by October 09, 2025, confirmation and evidence that the daily play space check is being conducted and logged.
