Observation. A new Manager has been employed, and the Licensee has not notified Licensing. The Licensee is required to immediately notify Licensing of any changes related to the Licence including the change of Manager. Submit by August 8, 2025 the required notification of change to Manager.

Observation. Children had access to hazardous item such as cleaning products. Having access to hazardous materials poses a risk to the health, safety and wellbeing of children in care. The door to the room containing the cleaning products was closed by the Manager, this contravention was corrected during the inspection.

Observation. There are no records of minor accidents since July 2024. Employees confirmed that children had been provided with basic first aid but there was no record of the accident. The Licensee is required to keep a record of minor accidents. Submit by August 11, 2025 confirmation and evidence that all minor accidents are being recorded. Child records were reviewed. Two records were found to be missing information such as, name and phone number of the child's medical practitioner, and information on who does not have access to the child. Submit by August 11, 2025 confirmation that all children's records now have the required information. Employees do not record the amount of medication administered. Employees are required to record the amount given. Submit by August 11, 2025 confirmation and evidence that employees will record the required information each time medication is administered. The fire drill and emergency plan practice log for 2024 was not available to be inspected. The Licensee must ensure records identified in Child Care Licensing Regulation section 59 are kept in a single place and can be retrieved within a reasonable time. Submit by August 11, 2025 confirmation that these records are at the facility.

Observation. A child's registration record states there is a court order in place. The court order has not been provided, making it difficult for staff to prevent this person from accessing the child.

Observation. It was alleged that children were not being appropriately supervised outdoors, and were engaging in unsafe behaviour. The Manager explained the supervision practice where staff supervise both the indoor and outdoor space from the doorway. Licensing observed the outdoor play space from the doorway and determined that this is not effective supervision as it is not possible to see all parts of the outdoor play area, including behind the storage shed where children are said to be engaging in unsafe behaviour. - Substantiated.

Observation. Contravention identified during the investigation: Licensing observed two sections of portable outdoor fencing with gaps large enough for children to go through. Therefore, the outdoor play area is not fully enclosed.

Observation. Contravention identified during the investigation: - The Licensee did not notify Licensing of the change of Manager. The Licensee informed Licensing of the Manager Change on 30-Nov-2021 to be effective from 01-Dec-2021.

Observation. The complaint stated: - Three children were not collected from the bus stop after an early release, and were left with no supervision from the Thrive Out of School Care - Enderby staff. It was confirmed that the Thrive Out of School Care - Enderby staff had been requested and agreed to the early pick up via text message communication. The text message included both the date and the time of the early pick up – Substantiated. The complaint stated: - A child was observed outside bouncing a ball for several minutes. No staff were outside with the child. Thrive Out of School Care –Enderby staff said children are not allowed outdoors without a staff member, and while they did not recall this event, they said they would have been standing in the doorway monitoring the child - Unable to substantiate.

Observation. Contravention identified during the investigation: The Licensee did not report the missing children to Licensing within 24 hours of becoming aware of the reportable incident.

Observation. Contravention identified during the investigation: - Licensing observed the Thrive Out of School Care - Enderby staff and children walking from the school back to the facility. Staff did not implement the facilities policies. Children were observed running ahead of staff while staff walked together. This does not align with the facilities policy, that states, “enforce that they (children) stay behind the front leader and infront of the back leader.” “Don’t leave walking gaps bigger than 6feet (2metres)”.

Observation. - Licensing observed the name of a new manager posted in the facility. Licensing was not notified of this change as required.
- C10.2 - Are children supervised, and are employee to children ratios maintained?Critical
Observation. - Upon arrival at the facility to conduct the inspection, licensing observed children arriving at the facility on foot with no staff person in clear sight. - Licensing observed one staff person proceed to the area where the washrooms were located while the other staff person was outside with some of the children. Other children in care who remained inside were left unsupervised in the main room for approximately 5 minutes. - Licensing observed 18 children (including children in Kindergarten), being cared for by one staff person at the facility. The second staff person stayed behind alone at the school where pick up occurred with one child in their care. The second staff person arrived at the facility with the one child in care approximately 15 minutes later.

Observation. During the inspection, record keeping systems for staff files were reviewed by licensing. Licensing discussed with a staff person that the Occupational first aid certificate observed in the new manager's file was not an accepted first aid course as per what is required in Schedule C of the CCLR. - The staff person (not the new manager), who was caring for a child alone at the school where pick-up occurred, did not have a valid first aid certificate.

Observation. During the inspection, child record keeping systems were reviewed by licensing. - Based on the information received from the staff during the inspection, it was determined that the licensee did not have a system in place for documenting minor accidents, illnesses and unexpected events as required. - Based on the information received and reviewed during the inspection, it was determined that the children's electronic records did not contain "written consent" from parents to call an ambulance for their child in the event of an emergency.

Observation. - Licensing observed pieces of wood with exposed nails accessible to the children in the outside play area. - Licensing observed the children having free access to the outside storage shed. Licensing observed children in care tripping over items such as the bases for some of the fencing panels while retrieving play items from the shed. As well, licensing observed some larger items, such as hockey nets, stored precariously in the shed. - Licensing observed hazardous products accessible to the children in an open storage cupboard in the main room. - The staff person, who was caring for a child alone at the school where pick-up occurred, did not have a first aid kit readily accessible nor the emergency card information readily available for that child.
