Observation. Fire drills have not been practiced monthly by employees since the facility opened. Every employee is required to practice implementing the fire drill system at least once per month to be prepared in the event of an emergency. A response is required by December 1, 2025 to confirm the fire drill has been completed.

Observation. Cleaning solutions were observed on the counter and under the sink within reach of the children. Cleaning solutions are considered a hazardous substance and may pose a risk of injury to children. A response is required by December 1, 2025 to confirm the risk has been mitigated.
