  • C10.2 - Are children supervised, and are employee to children ratios maintained?Critical

    Observation. It was alleged that children had attended the program prior to school starting in the summer of 2025 when they were not eligible to commence care until September 2025. Through a review of two children's records and attendance records the children did attend the program in July and August prior to when they were eligible to receive care. It is the responsibility of the Licensee to ensure the ages and requirements of children in care align with what is outlined in Schedule E of the Child Care Licensing Regulation. A response is required by November 7, 2025 that outlines Schedule E will be maintained and followed at all times. Substantiated. It was alleged that the number of children attending the program occasionally exceeded the maximum child to staff ratio, leaving one employee with more children than allowed at the facility. Through a review of attendance records and children's files the number of children per day was observed to be recorded between 7 and 12. One staff person is allowed to have 12 children on their own as per the Child Care Licensing Regulation. Unsubstantiated.

  • C4.1 - Are facility records current and complete?Non-critical

    Observation. It was alleged that the children's files are incomplete and missing information. A sampling of files were reviewed and are missing: the sex of the child, immunization status of the child, start date the child commenced care, medical practitioner name and phone number and the medical services number for the child. This is a re-occurring contravention originally found out of compliance on March 18, 2024 and June 13, 2025. Children's files are required to be complete in order to maintain the health and safety of the child while in care and in the event of an emergency. Submit for review all children's files currently enrolled to confirm complete by November 7, 2025. Substantiated. It was discovered in relation to the complaint that all children's files reviewed are missing the consent to call a medical practitioner or ambulance. This is a reoccurring contravention originally found out of compliance on June 13, 2025. Children's files are required to have all consents in the event of an emergency. Submit for review all children's files currently enrolled to confirm complete by November 7, 2025. It was alleged that the documentation is missing to record outdoor play space inspections done by employees. The outdoor play space checklist was observed to be complete and up to date. All checks were conducted by the Licensee. Unsubstantiated. It was alleged that one past employee file contains character references that were obtained during the screening process of hiring the employee. A review of the person's file contained the character references as was required for employment. It is the responsibility of the Licensee to return the character references or destroy them once a person is no longer employed at the facility. A response is required by November 7, 2025 to confirm character references have been returned or destroyed as required. Substantiated. It was discovered in relation to the complaint that one child who required an Epi-Pen who is no longer receiving care did not have a corresponding care plan in place for the safe care of the child. A care plan is required to be developed for any child who requires extra support while in the care of the Licensee. A response is required by November 7, 2025 that outlines how care plans will be developed as required for children with medical needs.

  • C3.1 - Does the facility have sufficient employees who possess adequate training and experience to meet the care, supervision, and needs of the children?Non-critical

    Observation. It was alleged that fire drills were not conducted by all employees monthly. Information was documented in the minor incident log book that fire drills were discussed with employees each month and occasionally practiced. All employees did not practice implementing the fire drill system each month. This is a reoccurring contravention originally found out of compliance on March 18, 2024. Every employee must practice implementing the fire drill system at least once per month to be prepared in the event of an emergency. A response is required by November 7, 2025 that outlines how fire drills will be conducted as required. Substantiated.

  • C9.1 - Are there plans and actions in place to ensure medications are stored and administered properly?Non-critical

    Observation. It was alleged that one child who had an anaphylactic allergy to peanuts and required an Epi-Pen did not have any corresponding documentation to support the allergy on file. It was observed on the child's registration form that the allergy was noted by the parent and that an Epi-Pen was required. Evidence was not available that the License had agreed to give the medication to the child or that the parent has prescribed the medication for administration at the facility. Any medication that may be administered at the facility requires permission from the parent/guardians as well as evidence the License has agreed to administer the medication. Substantiated. It was alleged that a child who required an Epi-Pen had attended the facility without the medication available for use if needed. The Licensee states that the parent verbally disclosed that the child only needed the Epi-Pen if the child ingested peanuts so the child did not require the Epi-Pen regularly at the facility. An anaphylactic allergy to peanuts requires an Epi-Pen to be present at the facility at all times when the child is in care. Having the life saving medication on site will ensure the health and safety of the child at all times. A response is required by November 7, 2025 that identifies how this risk will be mitigated for all children who require medication. Substantiated. It was discovered in relation to the complaint that the child who required an Epi-Pen (when the medication was present) had kept the medication in the child's backpack within reach of the other children. It is the responsibility of the Licensee to ensure that medication is not accessible to the other children in care to maintain the health and safety of all children attending the facility. A response is required by November 7, 2025 that outlines how medication will be stored as required.

  • C7.1 - Is the facility safe, sanitary, free from hazards, in good repair, and appropriate for the age and development of children?Non-critical

    Observation. It was alleged that the emergency bag that is taken off site for the program is inadequate and missing first aid supplies. The Licensee confirmed that band-aids and tissue are the only first aid supplies that are regularly taken off site for the program in a fanny pack. The items as listed are not sufficient first aid supplies to be considered a first aid kit for use by employees. Complete first aid kits are required to be readily accessible in the event of an emergency including while care is provided off the premise. A response is required by November 7, 2025 that outlines how first aid kits will be accessible at all time for employees. Substantiated. It was alleged that repairs are not being made to damaged play structures as identified during outdoor play area safety inspections by employees. Previous logs noted in the outdoor play space checklist identified repairs that were needed for an outdoor play structure. A full inspection was conducted for the outdoor play area and structures as part of the investigation. No damages or hazards were observed to be present, thus indicating the previous concerns had been mitigated as required. Unsubstantiated. It was alleged that one out of two available washrooms for children is kept inaccessible for the children's use even in the event of emergency. Evidence of use was observed during inspection in both of the washrooms available to the children. Paper towel was observed in the garbage cans in both washrooms indicating the washrooms have been recently used. Unsubstantiated.

    Observation. The Licensee and the one other employee at the facility do not have a valid first aid and CPR certificate. The Licensee's certification expired on June 6, 2024. A Licensee must ensure that children have at all times immediate access to an employee who holds a valid first aid and CPR certificate in the event of an emergency.     Observation. One bicycle is available for the children to ride in the outdoor play area. Children were observed riding the bicycle without a helmet. It is the responsibility of the Licensee to ensure proper safety equipment is available and used to prevent injury in children.     Observation. It was alleged that that Licensee is providing care for a child for more than 13 hours per day. Through interviewing the Licensee it was confirmed that there have been occasions where a child has been in care for more than 13 hours per day. Outline the system developed to ensure any child is not being provided care for more than 13 hours per day. Allegation is Substantiated. It was alleged that the Licensee conducted inappropriate behavioral guidance with a child resulting in physical restraint. Through a review of the information provided by the complainant and interviews with the Licensee and facility employee, it has been determined on the balance of probability the allegation is unable to be substantiated.

    Observation. Through a review of the minor accident, illness and unexpected events log book an incident involving unusual or aggressive behavior of a child was noted to have occurred that was not reported to licensing as required. This contravention is not directly related to the complaint but found as a result of inspection. Licensing is to be notified within 24 hours after a child is involved in, or may have been involved in a reportable incident. Submit the report as stated above to Licensing. Outline the system developed to ensure all incidents as described in Schedule H of the Child Care Regulation are reported to Licensing within 24 hours as required. It was alleged that the facility does not have a care plan developed for a child who requires extra support. Upon inspection of a child's care plan, it is noted that the plan is complete as required. Allegation is unsubstantiated.

In reviewing employee files, it is noted that one employee's file is incomplete missing: a cleared criminal record check in the facility name, a record of the person's work history, evidence the person has complied with the Province's immunization control program.
