    A licensee must keep, for each child, a record showing the following information: (a) name, sex, date of birth, medical insurance plan number and immunization status. Observation (CORRECTED DURING INSPECTION): Upon inspection, Licensing observed the files to be missing the child's gender.

    Corrective action. The staff explained the newer forms were missing the child's gender. The staff updated the files and said the master copy would be updated.

    A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.

    Observation. Upon inspection, Licensing observed: -a couch in disrepair exposing foam -the top of the chain link fence located in the outdoor play area is in disrepair exposing sharp areas which poses a hazard -gaps in the wooden panels of the fence located in the outdoor play area. This is a repeat contravention.

    Corrective action. Please ensure that furniture, equipment and fixtures within the facility are clean and in good repair.

    A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.

    Observation. Upon inspection, Licensing observed the following: -Children's bedsheets and mats were touching, posing a cross contamination risk. Staff stated that children's bedsheets are not washed daily. -The wire plugged into the outlet of the CD player in the play area is accessible to children. -The TV and VCR located on the table in the nap area is not secured and poses as a safety hazard. -The chicken wire around the tree in the School-Age program playground is coming apart, exposing sharp areas to children. -There are mushrooms growing in the School-Age program's outdoor playground posing a health hazard. -The window leading to the kitchen was open while children were playing outside, posing a falling hazard with a high drop.

    Corrective action. Please ensure that a healthy and safe environment is provided at all times.

Upon review of medication, it was observed: -One child requiring life saving medication had no written direction regarding when to administer medication. -One child requiring life saving medications did not specify reactions, or clarify which out of the two medications provided should be administered.

    Corrective action. Please ensure that all children that require medication have written directions specified by the parent or care plan regarding the administration of medication.
