  A licensee must keep, for each child, a record showing the following information: (a) name, sex, date of birth, medical insurance plan number and immunization status.

  Observation. Four children's files were reviewed. Three were missing MSP numbers. Two were missing immunization status.

  Corrective action. The licensee will obtain MSP numbers and immunization status for the files that are missing them.

• RECORDS AND REPORTING

  A licensee must keep, for each child, a record showing the following information: (c) daily attendance record, indicating for each day whether the child is absent or, if the child is present, the time of arrival and departure. Observation (CORRECTED DURING INSPECTION): There were 6 children present; only 1 child was signed in.

  Corrective action. Ensure to keep all children's current records, i.e. time of arrival and departure.

• EQUIPMENT AND FURNISHINGS

  A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.

  Observation. LO observed and noted that diaper changing mat started to rip. A discussion was had to use mat free of rips and tears for proper sanitization purposes after each diaper change.

  Corrective action. Ensure that changing pad is clean and in good repair.
