Observation. Licensing reviewed 6 children's files and the following was observed: - three children's files were missing the gender of the child. - one child's file was missing the MSP number.
Corrective action. Ensure children's files are complete.

A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.
Observation. Licesning observed that there were some medication on the table which was accessible to children.
Corrective action. Substitute staff removed the medication and stored it at the higher shelf.
11010 - CCLR s.13(1) - EQUIPMENT AND FURNISHINGS
A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.
Observation. Licensing observed the following: - Fixtures such as the pillar in the playroom has the wall paper coming apart. -The rubber taping on the edges of fire place hearth is coming off. -The floor in the playroom needs deep clean as there is some tape on the floor. - Too many toys and clutter in the playroom. - Heat vent in the hallway needs repair. - Areas of the facility were not clean such as dirty socks on table, water bottles and slippers underneath the sheves and sink vanity in the playroom. - The broom to be removed from playroom.
Corrective action. Licensee will ensure and provide the plan to keep up with the cleanliness and fixtures in good repair.
11020 - CCLR s.13(2) - HYGIENE AND COMMUNICABLE DISEASE
A licensee, other than a licensee who provides a care program described as either Preschool (30 Months to School Age) or Group Child Care (School Age), must have in a community care facility a sturdy surface for changing diapers, located outside the food preparation area, next to (b) a wash basin.
Observation. Licensing observed the diaper mat was covered with the cloth and diapers were being changed in the kitchen corner of the facility.
Corrective action. Licensee will change the diapers near the bathroom area, will remove the cloth sheet and use the bleach solution to clean after each use.

Observation. Substitute staff were not able to locate the Emergency supplies kit
Corrective action. Licensee will place the kit in a visible location so that it is easily accessble.

A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.
Observation. Licensing observed the following: --Fireplace hearth has sharp edges around it. --The bedding sheets used by children were stacked together in the closet promoting cross contamination. --The basket full of plastic toys was stored on top of the play kitchen that fell on the floor close to where children were playing. --Licensing observed many heavy wooden toys placed in lower shelves that are easily accessible by toddlers and can fall on them if pulled. --Storage closet in the hallway next to the bathroom was not locked
Corrective action. Please ensure that a safe environment is provided at all times while children are under the supervision of employees.
11010 - CCLR s.13(1) - EQUIPMENT AND FURNISHINGS
A licensee providing a care program to any child younger than 36 months of age must provide a separate sleeping area located away from any activity area.
Observation. Licensing observed that the nap area is in use while children are in care and required nap. Licensing discussed that its licensee's responsibility to make sure that the nap area is available for staff to use as needed by the children.
Corrective action. Children are to be provided with a separate nap area which is clean and safe for use away from the activity area, a checklist has been put forward as a method of ensuring a safe environment however it is noted to be ineffective as observed by licensing and mentioned by the staff. The room was also in use while children were in care and required nap. Please outline your plan to address continued room use at all times ( during operating hours).

A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.
Observation. Overall level of cleanliness and organization of the materials in the facility requires attention. Licensing observed the following: --Bathroom needs deep cleaning- debris i.e dust balls, used toilet paper rolls, soiled toilet and soiled bathroom flooring. --Garbage cans are kept by the front entrance of the daycare and are overflowing with garbage. --Used pop cans and debris was observed by the front entrance of the daycare door.

Observation. Single use towels or soap is not available to children to wash and dry their hands after hand washing in the bathroom.

A licensee must ensure that children do not have access to any object or substance that may be hazardous to the health or safety of a child.
Observation. Licensing observed open garbage bag and open ice melt bag at the entrance to the facility. Dog feces were observed throughout the children’s’ outdoor play area. Also noted large rocks,plastic debris and pile of wet leaves in the play area. Upon inspecting the indoor area it was observed that the plastic bags,brooms,mop were stored by the sink that are easily accessible to children.
Corrective action. Ensure that children do not have access to items which present risk to their health and safety. The dog feces have been an ongoing issue which requires a permanent solution. This is a Repeat Contravention DCAR-BY5NGJ. Please submit a plan to ensure that children's outdoor play area is free of hazards on a regular and ongoing basis.
11200 - CCLR s.17 - EQUIPMENT AND FURNISHINGS
A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.
Observation. Plastic bags were stored by the sink that are accessible to children. The bedding sheets used by children were all stacked together in the closet promoting cross contamination There were many toys i.e puzzle pieces,blocks,playing cards and other toys on the floor which are a tripping hazard. During the inspection a child ran and slipped on piece of plastic food and hit his head on the floor.
Corrective action. Please ensure that a safe environment is provided at all times while children are under the supervision of employees.

A licensee providing a care program to any child younger than 36 months of age must provide a separate sleeping area located away from any activity area.
Observation. At Arrival the Licensee was observed returning from the nap room with a child ( 2.5 years ) in her arms and a family member also exited the room and went upstairs. It was noted that a large TV displayed video games on the screen while the child was in the room. The room was not in the condition as set out in the Naproom / Shared Space Checklist as put forward by the licensee and included cords on the floor.
Corrective action. Children are to be provided with a separate nap area which is clean and safe for use away from the activity area, a checklist has been put forward as a method of ensuring a safe environment however it is noted to be ineffective as observed on 2 separate occasions. The room was also in use while children were in care and required nap. Please outline your plan to address continued room use at all times ( during operating hours)

11020 - CCLR s.13(2) - EQUIPMENT AND FURNISHINGS
A licensee must ensure that children do not have access to any object or substance that may be hazardous to the health or safety of a child.
Observation. Observed children playing outdoors , dog feces located through out the yard ( 3 large breed dogs within the home ) Licensee developed a plan to address this which as observed is unsuitable. Step 1 - Children asked to wait on the concrete while the licensee removes the feces ( children instructed to play in an area , this area also contained feces which children had access to without supervision while the licensee was attempting to scoop the piles.) Step 2- spraying down the area (not completed as the hose was not working) Handwashing after yard clean up did not take place. Also noted large rocks balanced on the top of a fence panel , these were removed by the licensee when pointed out as a hazard. Garbage and cigarette butts located throughout the yard. Gate to the upper deck area appears broken no longer restricts access to the stairs - deck contained piles of cigarette butts and lighters. Licensee stated no one smokes within the home and was unclear how they were located there.
Corrective action. Ensure children do not have access to items which present risk to their health and safety. The yard has been an ongoing issue which requires a permanent solution. Forward your plan to include how you will ensure children have regular and ongoing access to a safe and secure area which is free from hazards.

Observation. Attendance records reviewed , time of departure is not captured.One child is not included within the attendance record.
