  A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.

  Observation. DLSOP Safe Play Space – There is no system in place to ensure regular checks and maintenance of equipment.

  Corrective action. The manager said they will consider a plan and provide a written response.

  When advertising or otherwise offering services to the public, a licensee must identify the care program being offered.

  Observation. A posted parent letter/notice for families included the name preschool. The attendance records showed more than 30 children registered for 2 hour and 4 hour classes 2 days per week, both pieces of information support that the Group Child Care (30m-SAC) classroom is operating as a preschool program. This is a repeat contravention. (Photos taken and a copy was provided to the manager).

  Corrective action. The manager said they will consider a plan and provide a written response.

  A licensee must keep, for each child, a record showing the following information: (c) daily attendance record, indicating for each day whether the child is absent or, if the child is present, the time of arrival and departure.

  Observation. -Attendance for at least one child present in Classroom A was signed into Classroom B. Repeat contravention.

  Corrective action. The manager said they combine groups in the morning, and will accurately document children moving forward, and provide a written response with this plan.

  A licensee must display in a prominent place in the community care facility both the licence, including any terms or conditions of the licence, and the name of the manager.

  Observation. Manager's name was posted next the name of a staff person who is not the manager.

  Observation. Sign next to the door, registration forms, and schedules in the program said preschool. Group Child Care (30m-SAC) is the only care type offered.

  Corrective action. Please ensure to adjust all documents, and signage to reflect the care type provided.

  Without limiting subsection (1), a licensee must ensure that (b) the ratio of employees to children attending a community care facility is no less than that permitted in Schedule E.

  Observation. On Friday afternoons from 4pm-5pm a dance class is offered to children who attend the program. On at least one occasion there were 21 children in the classroom licensed for 16 children, with 1 ECE staff, and two dance teachers.In the second classroom there were 3 children with an ECE staff.

  Corrective action. Licensee will consider a plan to ensure compliance, and provide a written response.

  Corrective action. Licensee will consider a plan and provide a written response.

  Observation. Licensing reviewed 5 employee files, of those one employee had no record of the person's work history in the file.

  Corrective action. Licensee will consider a plan and provide a written response.

• EQUIPMENT AND FURNISHINGS

  A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.

  Observation. -The cubbies located in both classrooms were not secured to the wall, providing risk for tipping. -One toilet paper dispenser was broken leaving a hole in the wall next to the toilet. -Plastic bags were located in a cupboard under the sink in the GCC(30m-SAC) program (corrected during the inspection). -no gate at the top of cement stairs leading to the GCC(30m-SAC) classroom.

  Corrective action. The licensee has provided licensing a plan to fix the wall in the bathroom and anchor the cubbies by May 31, 2024

  A licensee must ensure that any heated water accessible to children is heated to no more than 49° Celsius. Observation (CORRECTED DURING INSPECTION): Of four sink water temperature measured, one sink measured at 53, 5 degrees Celsius.

  Corrective action. Corrected during the inspection.

  Without limiting subsection (1), a licensee must ensure that (a) children attending a community care facility are divided into groups, with each group having no more children than that permitted in Schedule E.

  Observation. During the inspection it was noted that there is a tutoring program that occurs monday-friday from 3:30-5:30pm in the GCC(30m-SAC) program. During this time the children enrolled in GCC(30m-SAC care are combined with the Preschool program from 3:30-3:45pm in the preschool classroom. On review of the attendance and enrolment records, the group was noted to be regularly combining the group, exceeding the sizes in Schedule E and is mixing care types in the classroom.

  Corrective action. Licensee will provide a plan, that includes moving the tutoring class to outside operational hours.

  A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (a) a criminal record check for the person.

  Observation. Licensing reviewed 5 employee files, of those one file contained an expired criminal record check. The licensee said they had obtained the criminal record check however was unable to find a copy.

  Corrective action. The licensee will contact the criminal record review program and obtain a record of the current criminal record check.

  A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.

  Observation. During the inspection licensing observed: -Damage to the wall where drywall was exposed in 6 small areas of play. Some areas were patched, and staff said they were planning of painting in the coming weeks. -In one bathroom for children's use, the toilet paper roll was broken off the wall

  Corrective action. Please ensure to establish systems to ensure that a healthy and safe environment is provided at all times.

  Observation. Five staff files were reviewed, of those, one staff certification had expired. The staff said that they had submitted the renewal one month before but had not yet received it.

  Corrective action. Please ensure to display in a prominent place in the community care facility the certificate or a copy of the certificate.

  Observation. At licensing arrival the group child care program and the preschool program groups were combined, and playing outside. There was no record of children being signed in and out of each classroom. This is a repeat contravention.

  Corrective action. Please ensure to create and implement a system that reflects which program they are attending and the time of arrival and departure.

  A licensee must keep, for each child, a record showing the following information: (a) name, sex, date of birth, medical insurance plan number and immunization status. Observation (CORRECTED DURING INSPECTION): Licensing reviewed 20 children's files, of those two did not contain record of the child's gender.

  Corrective action. Please ensure to establish a system to ensure record showing gender of the child is documented.

  Observation. Licensing reviewed 5 staff files, of those, one staff's file contained an expired criminal record check. Staff in the program said that the criminal record check had been completed however it was not available to review during the inspection.

  Corrective action. Please ensure to keep record of criminal record check for persons employed in the facility

  A licensee must ensure that any heated water accessible to children is heated to no more than 49° Celsius.

  Observation. Licensing observed that the water temperature in a bathroom located in the Preschool room was reading over 51 degrees celcius, measured with a digital thermometer. The staff said that children generally use the other bathroom with one sink and one toilet, where the hot water is shut off.

  Corrective action. Please ensure that hot water, accessible to children in care, remains below 49 degrees celcius.

  A licensee must ensure that children do not have access to any object or substance that may be hazardous to the health or safety of a child.

  Observation. Licensing observed that a cleaning solution was located in a cupboard under a bathroom sink. This bathroom is primarily used for staff but is included in the facility requirements for the group of 20 preschool children.

  Corrective action. Please ensure any cleaning solutions/products remain inaccessible to children in care

  Observation. Licensing observed that the licence was not posted in the Group Child Care (30-SA) program , nor was the name of the Manager posted in any of the programs.

  Corrective action. Appropriate staffing must be in place to meet the staff to child ratio of the group. Please submit a corrective action plan.

  A licensee must keep, for each child requiring extra support, a current care plan showing the following information: (a) the diagnoses relevant to the child's requirement for extra support, as made by health care professionals.

  Observation. No care plan available for a child requiring extra support and to be developed in consultation with parents/family.

  Corrective action. Please ensure a care plan is available to support staff in managing and meeting the child's needs.

  A licensee must keep, for each child, a record showing the following information: (c) daily attendance record, indicating for each day whether the child is absent or, if the child is present, the time of arrival and departure.

  Observation. Licensing observed that the attendance records for the Preschool program showed 3 children in attendance that were actually attending the Group Child Care (30-SA) program. These children attend for a half hour prior to the daycare staff arriving, but were not signed out. Discussed appropriate methods for accurately reflecting the information.

  Corrective action. Please ensure attendance records accurately reflect children's movements between programs.

  Observation. On review of four staff files, Licensing observed an expired criminal record check for the Manager. Staff said that the Manager has recently worked as a staff at the facility.

  Corrective action. Please ensure all staff have a current cleared criminal record check.

  A licensee must not employ a person in a community care facility as (a) an educator or an assistant unless the person holds a certificate issued under Division 2 [Employee Qualifications], which must be verified by the system maintained by the early childhood educator registry. (Educator or assistant certificate validation)

  Observation. On review of four staff files, Licensing observed that two were missing current validations.

  Corrective action. Please ensure a current validation is available for all certified staff.

  A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.

  Observation. One play yard gate, leading to the side yard, was unable to latch closed, and the other gate, leading to the exit of the premise had screws coming loose, causing difficulty securing the latch shut.

  Corrective action. Please ensure the gates are in good repair, while children are in attendance.
