  Observation. Licensing observed a log of all inspections, maintenance, and repairs was not recorded in June and July, 2026. Licensee stated that they have been checking the play space but forgot to record it on the log.

  Corrective action. Ensure a record of all inspections, maintenance, and repairs is kept.

• A licensee must keep, for each child, a record showing the following information: (a)name, sex, date of birth, medical insurance plan number (MSP number does not apply to REC only) and immunization status.

  Observation. Licensing audited four children's files and one file was missing a record of medical insurance plan number.

  Corrective action. Ensure all children's files contain a record of medical insurance plan number.

• A licensee must keep, for each child, a record showing the following information: (d) name and telephone number of a parent, medical practitioner (doctor's name and number does not apply to REC only) and emergency contact

  Observation. Licensing audited four children's files and one file did not contain a record of a name and telephone number of a medical practitioner.

  Corrective action. Ensure all children's files contain a record of a name and telephone number of a medical practitioner.
