  Observation. Licensing observed a mouse wandering in the facility hallway. Licensing also observed that the Corrective Action Plan developed in response to the 2024 complaint intake regarding health and hygiene practices was not being followed. The weekly and monthly cleaning checklists were not documented. The manager stated that the weekly cleaning checklist was not prepared for the week of August 3 to 7, 2026.

  Corrective action. Ensure to operate the facility in a manner that will promote the health and safety of children in care.

  Observation. Licensing observed a volunteer did not have a valid CRC.

  Observation. Licensing observed the December 2025 and March 2026 monthly fire drills were not recorded.

  Observation. Regarding intake May 29, 2026. Through the investigation process, Licensing determined that a care plan had not been developed in consultation with the child's parent until May 29, 2026, despite the child having demonstrated unusual or aggressive behaviour.

  Corrective action. Ensure to develop the care plan in consultation, with a parent of the child requiring extra support and any person requested by the parent. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: On July 6, 2026, Manager submitted a corrective action plan to Licensing. The corrective action plan included: care plans will be developed promptly in consultation with families when a child demonstrates unusual or aggressive behaviour, reviewed regularly to ensure they reflect current needs, and updated as required. The corrective action plan was reviewed and accepted by Licensing.

• A licensee must keep (a) in the case of employees, all records required under section 19 (1) [character and skill requirements] for the entire time that the subject of the records is an employee of the community care facility,

  Observation. Licensing observed that, of the ten staff working in the community care facility on this date, two staff were missing records required under section 19(1).

  Corrective action. Ensure that, in the care of employees, all records required under section 19(1) are kept for the entire time that the subject of the records is an employee in the community care facility.

• Applicants for licences and licensees must notify a medical health officer immediately of any change in the information provided under section 9

  Observation. Licensing observed that the Group Child Care (School Age) Room 14 was no longer operating a Group Child Care (School Age) program. Licensing observed that the washrooms accessible to children in the Licensed Group Child Care (School Age) Dolphin and Orca rooms did not have access to the washrooms due to costodial services.

  Corrective action. Ensure to notify a medical health officer immediately of any change in the information provided under section 9.

  Corrective action. Ensure the name and number for a medical practitioner is obtained for children's files.
