    Observation. Licensing observed a mouse wandering in the facility hallway. Licensing also observed that the Corrective Action Plan developed in response to the 2024 complaint intake regarding health and hygiene practices was not being followed. The weekly and monthly cleaning checklists were not documented. The manager stated that the weekly cleaning checklist was not prepared for the week of August 3 to 7, 2026.

    Corrective action. Ensure to operate the facility in a manner that will promote the health and safety of children in care.

    Observation. Regarding intake May 29, 2026. Through the investigation process, Licensing determined that a care plan had not been developed in consultation with the child's parent until May 29, 2026, despite the child having demonstrated unusual or aggressive behaviour.

    Corrective action. Ensure to develop the care plan in consultation, with a parent of the child requiring extra support and any person requested by the parent. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: On July 6, 2026, Manager submitted a corrective action plan to Licensing. The corrective action plan included: care plans will be developed promptly in consultation with families when a child demonstrates unusual or aggressive behaviour, reviewed regularly to ensure they reflect current needs, and updated as required. The corrective action plan was reviewed and accepted by Licensing.
