  Observation. Licensing audited five staff files and observed three had no record of First Aid certificates and one staff First Aid expired on July 31,2024. Manager stated that only one staff, that worked two days a week, had a current First Aid certificate. Three days a week the facility did not have any staff with a current First Aid certificate.

  Corrective action. Ensure children have, at all times, immediate access to an employee who holds a valid first aid and CPR.

The Licensee Contact stated that the children would not use that slide until it has been fixed. Full compliance will not be achieved until the plan has been implemented and completed.

  Corrective action. Ensure that play equipment is free from entanglements.

  Corrective action. Ensure that the lead in water corrective action plan is being followed and the community care facility operates in a manner that will promote the health, safety and dignity of persons in care.

On this date, June 8, 2022 Licensing detected an entanglement on the fireman's pole using the entanglement device. This represents an immediate risk to the health and safety of children in care and must be corrected immediately. Manager stated that the children will not use the fireman's pole until the entanglement can be repaired and approved for use by Licensing.

  Corrective action. Ensure that all indoor and outdoor play materials and equipment accessible to children are free from hazards.
