  Observation. Licensing observed two Responsible Adults (RAs) with a group of 24 children and one RA with a group of 20 children. An additional RA would be required for this grouping.

  Corrective action. Ensure that the ratio of employees to children attending the facility is no less than that permitted in Schedule E.

  Observation. Licensing performed an audit of eight children's files this date. Three files did not have written consent to release to someone other than the parent.

  Corrective action. Ensure children's files contain consent to release to someone other than the parent.

  Observation. Licensing audited five staff files and one staff file contained one character reference. Two character references are required for each staff file.

  Corrective action. Ensure all staff files contain two character references.

  Corrective action. Ensure that all staff files contain copies of diplomas/certificates/evidence of training/skills obtained for each employee.

  Observation. This item has been carried over from Routine Inspection # LMEH-BM52GJ dated February 24, 2020. On May 18, 2021, Licensing audited six staff files, one Responsible Adult was missing evidence of training and skills.

  Corrective action. Ensure evidence of training and skills is obtained for a person employed in a community care facility.
