  Observation. Licensing audited four children's files. One child's file did not have a medical insurance plan number.

  Observation. Licensing observed one child had medication that did not include a record for the amount and time for it to be administered.

  Corrective action. Ensure the amount and times are obtained to administer medication.

  Observation. Licensing observed the SPS DOLSOP log that did not indicate a daily record of playground inspections had been completed.

  Observation. Licensing audited three staff files. One staff file did not have a CRC.

  Observation. Licensing observed the climber in the outdoor play space measured 5'2". This height exceeds the DOLSOP equipment height for 5 feet for preschool children (30 Months to School Age).

  Corrective action. Ensure equipment for preschool children (30 Months to School Age) does not exceed five feet.

  Observation. Licensing inspected the minor incident log book and observed there was a reportable incident on April 23, 2024 that was not notified within 24 hours to the Medical Health Officer.

  Corrective action. Ensure MHO is notified within 24 hours if a child has been involved in a reportable incident described in Schedule H.
