3070 - CCLR 57(2)(i) - For prevention of entrapment, spaces (i.e., holes/openings) must be smaller than 9 cm (3.5 inches) or larger than 23 cm (9 inches). Use Head & Torso test probes
Observation. Licensing observed a four foot indoor climber had cutouts on the top platform that measured 4.5 inches.
Corrective action. Ensure that spaces (i.e., holes or openings) are smaller than 9 cm (3.5 inches) or larger than 23cm (9 inches) to prevent entrapment.
3000 - SPS DOLSOP (Entrapment) - A licensee, other than a licensee who provides a care program described as either Preschool (30 Months to School Age) or Group Child Care (School Age), must have in a community care facility a sturdy surface for changing diapers, located outside the food preparation area, next to a wash basin
Observation. Licensing observed that a change table had been added into the infant play area. A washbasin was not located near the change table. Staff used the washbasin in the kitchen for hand washing.
Corrective action. Ensure a washbasin is located outside the food preparation area and near the sturdy surface for changing diapers.
