  Observation. Licensing audited six children's files and one file did not contain medical practitioner name.

  Corrective action. Ensure current records for each child contain a medical practitioner name.

  Observation. Licensing audited one care plan and it indicated it had not been reviewed since 2021.

  Corrective action. Ensure all care plans are reviewed at least once each year with the parent of the child and any person requested by the parent.

  Observation. Licensing reviewed eight children's files. One file did not contain a photograph of the child.

  Corrective action. Ensure a record is kept for each child with a photo or digital image of the child.

• For prevention of entrapment, spaces (i.e., holes/openings) must be smaller than 9 cm (3.5 inches) or larger than 23 cm (9 inches). Use Head & Torso test probes

  Observation. Licensing observed a four foot indoor climber had cutouts on the top platform that measured 4.5 inches.

  Corrective action. Ensure that spaces (i.e., holes or openings) are smaller than 9 cm (3.5 inches) or larger than 23cm (9 inches) to prevent entrapment.

  3000 - SPS DOLSOP (Entrapment)
• A licensee, other than a licensee who provides a care program described as either Preschool (30 Months to School Age) or Group Child Care (School Age), must have in a community care facility a sturdy surface for changing diapers, located outside the food preparation area, next to a wash basin

  Observation. Licensing observed that a change table had been added into the infant play area. A washbasin was not located near the change table. Staff used the washbasin in the kitchen for hand washing.

  Corrective action. Ensure a washbasin is located outside the food preparation area and near the sturdy surface for changing diapers.
