  Observation. There were items that are identified as, "keep out of reach of children" accessible to children.

  Observation. Employee records were reviewed; some were missing reference checks and immunization status. The person providing care and supervision for the first 30 minutes of the day is a Responsible adult, and does not hold the minimum required qualification of and Early Childhood Educator Assistant certificate.

  Observation. Some staff files were incomplete and had expired records. The Licensee does not have a criminal record check for the person who regularly drives the bus. There are no record of the emergency plan being practiced.
