    Observation. - The outdoor play area on the southwest side of the facility is enclosed by two gates: one leading directly into the play space and another along the pathway beside Rupert Street. At the time of inspection, the gates were not closing properly and had to be lifted to latch securely. As a result, the outdoor play area was not fully enclosed, allowing children potential access beyond the designated space. - Broken plastic with sharp edges is present along the pathway. - Two wooden panels on the south fence are loose and not in good repair.
