Observation. LO observed the following areas that must be addressed to ensure facility is clean, easy clean, in good repair and free from hazards: - step stools / tables and edging of counters in bathroom are wearing (and therefore unable to be sanitized). - dolls and barbies appears to need a deep cleaning - cardboard blocks are starting to wear and there appears to be of a limited amount for children to engage in deep play - outdoor space has some plants that are toxic to children - areas of bark mulch in path may require topping up as you are seeing the mesh for lining the garden - in the single bathroom, there is a air freshener spray that is accessible to children (in the unlatched cupboard)

Observation. LO observed 3 Care Plans posted. While two of the plans stated "use epi-pen", no epi-pens were available. The staff stated that one of the children does not require a epi-pen and the second has an expired epi-pen and parents stated they don't need it anymore. This was previously discussed with a licensing officer. Care Plans must have accurate detailed directions for staff to follow. It is also important to note that care plans must be reviewed annually with parents to ensure information you have is still correct. You can show this by having parents sign the form annually or requesting new forms from parents.

Observation. Children's files were reviewed with most being complete. There were a few that do not have parent AND emergency contact, one child has no dr. information or medical insurance information.

Observation. Fire drills are posted in the cubby area. Some months do not have noted fire drills. As noted above, drills must be practices and noted each month. It was also observed that emergency drill may not have been practiced in over a year.

Observation. The bathroom sinks in one of the bathrooms are starting to rust around the edges due to missing seals. Some of the laminate flooring by the bathroom has gaps between the planks presenting a tripping hazard.

Observation. Licensing observed a broken metal hook on the fence with a sharp edge. The top fence board showed green algae growth and is due for power washing.

Observation. Licensing observed two recent entries in the minor incident log book that would have been reportable (Nov 14 and Dec 7). Licensing discussed the entries with the manager.

Observation. Licensing observed two children use the bathroom and not wash their hands after. There are no direct sightlines from the activity space into the bathrooms. The lack off hand washing was not noticed by staff.

Furniture or equipment for sleeping is used by only one child at a time, is clean and comfortable, and is appropriate for the size, age and development of the children intended to use it. [CCLR Sec. 15(2)]
Observation. Licensing observed the mats stored upright against each other with sheets left on. Blankets and pillows were staked in piles on top of the mats. All mats had identical sheets and all blankets and pillows were identical. Mats, blankets and pillows were not labelled. Licensing inquired about cleaning practices and the staff stated that bedding is washed once a week. As items are not labelled or stored individual this means that children share mats, sheets and blankets. This poses a risk of cross contamination.

Observation. Licensing observed the following: 1) Adult metal gardening tools (metal claw, rake) in the corner of the yard accessible to children in care. A large hedge pruning tool in an unsecured shed accessible to children in care. Staff stated that the lock on the shed had broken. 2) An unsecured storage room. 3) Children had direct access to the staff room adjacent to the activity space as the door was wide open. Several children were playing unsupervised in this area.

Observation. Licensing observed the following on the outdoor playground: 1) Several fence panels were unsecured and easy to rock back and forth. One panel had a broken slat. The fence is not an effective, safe, and secure barrier. 2) A Kiddy Junction sign had been fastened to one fence panel with bolts. The bolts were protruding about 1" through the fence on the playground side posing a risk of injury to a child. 3) A large orange traffic bumper had been added to the playground beside the fence. The boulder enables children to gain a foothold and climb over the fence. 4) A plastic playhouse had been placed in front of the climber beside the slide. Licensing observed a child attempting climb on top of the roof as they held onto the climber railing. It is recommended to replenish the sand in the sandbox as levels appeared low.

Observation. Licensing observed metal chains along the fence and climber. The chains were looped and held together by metal locks. The chains appeared to be rusty and the loops were large enough to pose a risk of entrapment. Licensing was informed that these chains were once used to support hanging baskets for plants. Licensing recommends removing the chains and locks completely.
