    Observation. Licensing observed an expired Epi-pen for a child with an anaphylactic. During drop-off the Manager had a conversation with the family who agreed to bring a new Epi-pen into the facility.

    Observation. Licensing observed a broken toilet, the Manager stated that a work order has been put in place. Licensing also observed a lifted carpet tile in the gross motor room, the Manager stated that a work order has been put in place.

    Observation. Dirt and dust was observed a the base of the toilets and along the bottom of the tiles in the bathroom. The caulking at the base of the toilets has worn away.
