A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.

Observation. Indoors: - carpet is visibly dirty Outdoors: - grass was overgrown - wooden beams, cinderblocks, ladder and wheelbarrow accessible

Corrective action. Ensure the facility is clean and in good repair while children are in care.

A licensee must display in a prominent place in the community care facility the certificate, or a copy of the certificate, of any educator or assistant employed to work in the community care facility.

Observation. Manager's license was posted. Other staff's certificates were not. (repeat contravention)

Corrective action. Manager will post all staff certificates.

A licensee must not employ a person in a community care facility as (a) an educator or an assistant unless the person holds a certificate issued under Division 2 [Employee Qualifications], which must be verified by the system maintained by the early childhood educator registry. (Educator or assistant certificate validation)

Observation. Licensing reviewed three staff files, two did not have certificate validation. (repeat contravention)

Corrective action. Manager will obtain all certificate validation.

Observation. Facility exceeded the maximum capacity on more than one occasion.

Corrective action. Corrective action plan submitted

Observation. Physical restraint was used with a child during nap time.

Corrective action. Corrective action plan submitted

A licensee, other than a licensee who provides a care program described as either Preschool (30 Months to School Age) or Group Child Care (School Age), must have in a community care facility a sturdy surface for changing diapers, located outside the food preparation area, next to (a) a covered container for soiled clothing.

Observation. There is no covered container next to the diaper change area.

Corrective action. The Licensee will place a covered container in the washroom.

A licensee must display in a prominent place in the community care facility the certificate, or a copy of the certificate, of any educator or assistant employed to work in the community care facility.

Observation. The Licensee's certificate was posted. She does have substitutes regularly and their certificates are not posted.

Corrective action. Licensee will post all certificates of her staff.

A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (f) evidence that the person has complied with the Province's immunization and tuberculosis control programs.

Observation. Immunization status was not obtained for one staff.

Corrective action. Licensee explained she will obtain immunization status for this staff.

A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (c) a record of the person's work history.

Observation. One staff did not have a resume on file.

Corrective action. Licensee explained she will obtain a resume for this staff.

A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (a) a criminal record check for the person.

Observation. Two staff do not have a site specific cleared criminal record check.

Corrective action. Licensee explained she will obtain site specific checks.

A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.

Observation. The blind cords were hanging down

Corrective action. Ensure the cords are kept high and out of reach of children; there is a shelf below these windows

A licensee must ensure that children do not have access to any object or substance that may be hazardous to the health or safety of a child.

Observation. The following was observed: - rodent traps were in an unlocked cupboard in the nap area - cleaner was on the washroom counter

Corrective action. Ensure these items are not accessible to children

A licensee must establish a program to instruct children in, and to practise the rules of, health and hygiene.

Observation. One bed sheet appeard to look dirty and soiled. Another cot had bedding that was on top of each other.

Corrective action. Ensure that each child's bedding is stored separately

A licensee must keep, for each child, a record showing the following information: (a) name, sex, date of birth, medical insurance plan number and immunization status.

Observation. A random sampling of files were reviewed and one file was missing a care card number

Corrective action. Ensure to have this information on file for each child

A licensee must ensure that each employee (c) practices implementing the emergency plan at least once each year.

Observation. There is no evidence of the emergency drill for the year 2022

Corrective action. Ensure to practice and record this drill annually

A licensee must display in a prominent place in the community care facility both the licence, including any terms or conditions of the licence, and the name of the manager.

Observation. The name of the Manager is not posted.

Corrective action. Ensure to post the name of the Manager in a prominent place in the facility.

A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.

Observation. The gate for entry into the backyard off the driveway now latches to the lock, however the gate bends outwards at the bottom leaving a gap between the post and gate. A small child could put their arm through this area.

Corrective action. Ensure there are no gaps and the gate is sturdy and does not pose as a risk for children.

Observation. There was a bottle of lysol disinfectant on the toilet tank accessible to children

Corrective action. Ensure children do not have accesss to any substance that may be hazardous to them

Observation. - the blind cords were hanging down - the hose in the backyard was unraveled and is a tripping hazard - the gate gate leading to the backyard does not latch and lock

Corrective action. ensure a safe environment at all times when children are in care

Observation. There was no evidence that the 3 sink method is being used to wash the dishes; there were dishes in the drying rack but there is only 1 sink.

Corrective action. Ensure to use the 3 sink method if dishes are going to be provided by the facility; information has previously been provided

A licensee must (b) provide to employees and parents a written statement of the licensee's policy on behavioural guidance.

Observation. The Manager stated she does not provide parents with a copy of the policies and procedures

Corrective action. Ensure parents are provided with this policy

A licensee must have all of the following: (b) an emergency plan that sets out procedures to prepare for, mitigate, respond to and recover from any emergency.

Observation. There were no emergency supplies on site; discussion re: enough for 3 days

Corrective action. Ensure there are emergency supplies available on site

If a person prepays part of the cost of services provided by a class of community care facilities designated by the Lieutenant Governor in Council, the licensee must, at the time of the prepayment, deliver to the person a written statement setting out the terms and conditions on which a refund of all or any of the prepayment will be made.

Observation. The Manager stated she does not provide parents with a copy of the policies and procedures

Corrective action. Ensure parents are provided with the required policies

A licensee must keep, for each child, a record showing the following information: (c) daily attendance record, indicating for each day whether the child is absent or, if the child is present, the time of arrival and departure.

Observation. There are days when arrival and/or departure times have not been recorded

Corrective action. Ensure daily attendance recorded with arrival and departure times

A licensee must keep, for each child, a record showing the following information: (i) a photograph or digital image of the child, and other information that can be used to readily identify the child in an emergency.

Observation. 3 children's files were missing pictures

Corrective action. Ensure each child has a picture

A licensee must have in writing from a parent, and maintain at the community care facility, consent (a) to call a medical practitioner or ambulance in case of accident or illness if the parent cannot immediately be reached.

Observation. 2 children were missing emergency consent

Corrective action. Ensure each child has emergency consent on file

The licensee must not permit a person over the age of 12 to be ordinarily present on the premises of a community care facility while children are present, unless the person is of good character and the licensee has obtained a criminal record check for that person.

Observation. 2 criminal record checks were missing for individuals who live on the premise

Corrective action. Ensure anyone who is ordinarily present has a valid criminal record check on file
